Hey all !
In 2014 i was diagnosed with stage 2a (most probably) in the left testicle. They removed it and found lymph nodes smaller than 2 cm so they went with 2 cycles of bep.
2 weeks ago as i was doing my yearly check the doctor found something suspicious in the right testicle. They advised for Trying partial Orchiectomy and remove it while doing pathology during the operation. The tumor they suspected was really small (8mm) if i’m not mistaken.
so we did the operation and they found it was a seminoma again. They removed it and took another biopsy from the testicle. We also did CT scan.
Today the results came. CT scans were clear but the pathology came that they saw signs of precancerous activity in the biopsy.
that’s why now they are advising me to either do radiation or do a surgery again to remove it fully.
i asked if it’s considered a new primary cancer or if it’s the old one but he said they can’t really say this 100% but maybe it doesn’t matter.
we talked about testosterone replacement and he said it’s something not easy to live with all your life (i’m 31) and that you might be willing to take the risk and do only radiation.
The other thing i’m not sure about is how really radiation is effective. Will it kills cancer in the testicle 100% or is the risk higher that some develop later?
Anyone who has done any of them and know the side effect of both? Especially radiotherapy?
I’m not sure what to choose, it’s really a tough one. At the moment i would easily say just remove the testicle but i’m also worried that i would regret this in the future as i have to deal with the testosterone everyday.
it had been a crazy two weeks so far, i’m relived that it’s not too bad, and can only be hopeful from now on. I was really lucky that they found it. There was not even a lump.
Another thing to mention that this tine the tumor markers came back all normal this time (in 2014 it was very elevated, but the mass was huge back then)
any tips, recommendations would be extremely appreciated. You helped me a lot in the last days and also when i had the first cancer in 2014. I can’t thank everyone here enough.

So Sorry you find yourself here again.
I haven’t had radio therapy, so can’t comment on that.
I have had one testicle removed, and I have low testosterone, and am due to start replacement therapy. I suspect that if you keep the testicle, after radio therapy you may still need testestrrone. In which case you needlessly had radiotherapy.
To avoid future BEP, I think I’d have it removed. But it’s easier for me to say that as I don’t actually have to live with the decision.
Very good news that the surveillance picked it up so early that you have options open to you.
