MPV level 11.3 and Histology report

Good afternoon,

My husband had his robotic prostatectomy last Thursday and other than a scare on Friday (high temp and the sepsis protocol being put into action), it’s been better than expected.

I wondered if anyone could share some insight into the following questions, the first one is that one of the pre-op checks was to look at his MPV levels and they have come back outside of the normal perimeters, when googling it looks like it can be linked to blood cancer and bone cancer, but not prostate cancer. Can anyone tell me if they have come across this indicator before and what they were told or what it meant?

The other question I have is that, the surgeon told us after the prostatectomy that my husband had clear margins, but he also said the histology report if important because it tells you how aggressive you cancer was and future prognosis?

To me, this is confusing? If the margins were clear, what difference would the aggressiveness of the cells and stage make? (Genuine question) as they believe his cancer was localised to the prostate.

  • I'm not sure how much this helps Ang, but cancers are described as aggressive not just because of the speed of their evolution but also in relation to the cancer cells' ability to bind to one another to form a single mass. I have had this described to me as a cell's 'stickiness'. Cells are termed aggressive if they lose this quality and therefore have a tendency to travel within the body. A high Gleason score, such as the 9 that I have, indicates dangerous non-sticky cells which might be prone to drift away from the principal tumour site.

  • Hi Ang1404, That's good that the operation has been done and seems to have gone well despite the temperature spike.

    I don't know what MPV is but in terms of histology I had clear (negative margins) but was upgraded to Gleason 4+4 and the cancer had broken through the capsule. I also had intraductal cancer in the prostate ducts as well as the "normal" adenocarcinoma.

    This meant I was told my prognosis was not as good as if the cancer was contained and there wasn't any intraductal cancer.

    Histology is important as it can tell you if the cancer was completely contained and will also provide a more accurate Gleason score which can advise how things will likely pan out.

  • Hi Good news about the op, hope that this is the end of it.

    MPV levels (Mean Platelet volume) is  an indicator of platelet size, 11 is just at the top of the scale and should be monitored but what did the consultant say about it? Is he recommending further tests?

    regarding the margins etc, I don't remember his original stats but I think that u can have clear margins with an aggressive cancer if that's what u are asking?  Just that they caught it in time, what was the original Gleeson?
