More than one Tumour/Lesion in Prostate

We are back from our short holiday and hubby has his pre op assessment today for his Template Perineal Biopsy under a GA on Saturday.

I ordered the literature that was recommended by you lovely lot but nowhere is there any information on having two unconnected highly suspicious areas within the Prostate.

Can I ask if anyone else has experience of this and if this means they are connected or unconnected. One is in the peripheral area on one side and the other is midline and spans both sides. Anyone help would on this would be gratefully received.

Also for anyone who has had this type of biospy, how long were you uncomfortable/in pain for and we see it can caused swelling interfering in urine output. Hubby is a bit worried about this as he already has difficulty in going. Is it a real problem or just one of those things they tell you could happen, but in reality not many people get a problem.

Thanks for any help on these points.

    I had two suspicious areas/lesions when diagnosed, by the time I started treatment 4 years later  that had increased to 3 areas (different sizes).

    It sounds like he has 2 suspicious areas unconnected, do u have any idea of the size in mm?

  • Thank you for the speedy reply Steve. Both suspicious areas are 10mm each on the MRI scan and they have both been graded as PI-RAD 4

    The one in the peripheral area which can be felt on a DRE has been described as a prominence in a letter from the consultant to the GP. This was before the scan finding the second area.

    Similar in size to mine. I presume that the MRI shows contained within prostate so when u know the Gleeson score a plan can be put together for treatment.

    As u know normally surgery or RT.

    What is the psa by the way?

  • He has only had one blood test for his PSA and I think that was 550.

    We did ask when they told him he had two suspicious areas what the next steps would be if they confirmed cancer, only to be told that, that is a 45 minute appointment and there are too many variables to discuss in a 10 minute appointment. So we will wait for the results. He had spent 10 minutes discussing low and high grade tumours and needing to find out which they were. But he did throw on as we were walking out that they still might be benign!! 

    Yeah right, do you think we were born yesterday, even the nurse had put her arm round me, and given me a squeeze and asked if I was ok.

    Hence the research about what is what as they have already started some treatment as his urine output was, how did they put it, oh yes very unsatisfactory on all accounts.

  • Obviously that's a fairly high psa, is he already on hormone therapy or are they waiting on the biopsy first?

  • Nope, no hormone therapy. The consultant was adamant that they wouldn't do anything or discuss anything until the results were back which he warned us can take up to 4 weeks!

    As has been mentioned before, it is the waiting that drives you mad. You just want to know what is what. What your options are and to get on with it.

    They don't sound  very helpful. Does the MRI show that the tumours are totally contained within the prostate and nothing shows up outside the gland?

  • Ask away, but we really don't know a lot. We didn't even know about the PI-RAD 4 until we were copied into the letter to the GP. Hence all the questions. At the moment we are under the local hospital that doesn't even publish it's info on the number of ops the consultant does, success rates, which procedures are used etc. So we have been trying to read up on things, find things out etc so we have all the info to ask questions when we get the opportunity to.

    But to answer your question, no we don't know if they are contained, but presume one is as we got told it was buried, hence not finding it till the MRI which took one hour and a quarter after being told 30-40 minutes. I was climbing the walls in the waiting room.

  • Ok, just have to wait then but def worth asking  if  totally contained, I would imagine that they would tell u anyway but sometimes , not so sure.

    Psa of 500 , when was that done and have there been any other psa tests?
