PALB2 and Reconstructive Surgery

Helle, I’m Libby and I’m 22 years old.

My mother, her mother and both of my aunts have suffered Breast Cancer and I recently found out that both me and my two sisters have discovered we have the PALB2 gene defect. 

Sorry for the long question, I’m just hoping that someone has had a similar experience to me/understands what I’m asking!

My older sister has already had a double mastectomy and reconstruction surgery and I have decided to have the same next year, but there are some questions that she can’t answer for me regarding reconstruction (because she is much much smaller than me, in every sense!) 

For context, I am about 5’6, a dress size 10/12, but I do have a large bust, I’m a size 32F.
I had a short video consultation with a nurse about the surgery and something that really upset me about the conversation was that she said that I wouldn’t be able to have reconstruction for breasts the same size that I have now and that I could only possibly go up to a 32C (this nurse hasn’t been very factually correct in the past when dealing with my sister so I’m not sure how much to trust). I thought at the time that this doesn’t make sense as women have breast enhancements to be a very large size, and for my own mental health and self confidence I am certain that I would like reconstruction to be as similar to my own shape and size as possible. I understand that I don’t have very much knowledge on this area and I will be meeting my surgeon to discuss this very soon, but it is something that I am thinking about a lot recently and I would really like to put my mind at ease one way or another!

Would someone have any advice on this please and let me know if it is possible to have reconstruction to my own size?

    Hi Libby, how did you get on? I was diagnosed with breast cancer last November and currently getting chemotherapy 4 down 2 to go. I was also diagnosed with PALB2. I met with the plastic team yesterday and was told something similar. I’m a size 12/14 and there isn’t enough tissue to reconstruct the same volume I have now if I wanted the same as i have 32ff I would need to get implants and they were not keen on that because of the associated risks. Devastated doesn’t quite cover it my heads in a bit of a mess with it all if I’m honest. 

  • Hi there, I was due to have a mastectomy and DIEP reconstruction in March 2020, along came Covid and it was mastectomy only. Having gone almost a year flat on one side I’m ok with it but the prostheses is large and heavy...I’m a 36FF. Had an   appointment with my surgeon two weeks ago and had a chat about reconstruction next year. If I don’t want the DIEP I could go for an implant and reduction on the other side. Thinking about going for that.

    Just wondered if you had been offered a reduction along with implant, if not it could be  something else to consider.

    If I were you I would wait to see exactly what the reconstruction/plastics surgeon has to say. They have done hundreds of these procedures and know what can be achieved. Also, it depends on kind of reconstruction you are having - implant or DEIP. If you are having a DEIP you obviously need the fat available from somewhere to use, especially as you are are having a double mastectomy and reconstruction. I was told - admittedly I only had one side done by DIEP - that they weigh what is taken out and refill accordingly and then use their skill to achieve symmetry. I had and still have the same sized breasts as before and I don't have small breasts. I was skinny when I was 22 with no fat on my tummy but by the time I had my DEIP having had children it was a different matter!!! I would wait until you have had the meeting with the reconstruction surgeon. XXXXxxx
