Hi All. My husband was diagnosed with stage 4b in January 2025 and he had triplet therapy last year (and ongoing hormone treatment and darolutimide).
My husband’s PSA started rising a few months ago and a PSMA PET scan showed one (possibly two) areas of activity. He had radiotherapy on those spots 4 weeks ago and we just had a PSA test which has gone up slightly from the last time (4 weeks ago). Has anyone else had this? Would it take a while for the radiotherapy to actually bring the PSA down? Thanks in advance
Hi Anna42 , generally having a PSA test after 4 weeks isn’t very accurate. The lowest point (nadir) after RT is usually after a year. Don’t be overly concerned and I am sure the next test will be down.

I'm very sorry the triplet therapy hasn't worked as intended.
My understanding is that there can be what's known as "PSA bounce" following radiotherapy which many experience after treatment by external beam radiotherapy. This can occur for some considerable time and the lowest reading may not occur until around 18 months after.
Maybe try to have a word with his consultant or nurse specialist for reassurance if this is possible?
Things tend to happen slowly with prostate cancer.
