1st PSA result 6 months after finishing HT & 8 months after Radiotherapy of 1.9

Hello everyone,

Advice please ?

Hubby started HT in April/May last year & radiotherapy in June/July time (SBRT - 25 ish sessions).

So, he just called the hospital to get results of his first PSA test after his treatment. His pre-treatment PSA was 5.5, T3a & Gleason 3+4. The oncologist suggested the result would be indetectable at his first PSA test (at least that is what I recall she said) so I am concerned it is 1.9 when he called today. 

We have not had any further information just the PSA result - should we be concerned do you think or is this a normal range result ?

Any advice would be appreciated.

    Thank you for your post. I think I would be concerned with a PSA of 1.9.

    I know he had a low PSA to start with but being a T3a I would have thought the initial Hormone Therapy would have been longer than 6 months. After Radiotherapy you should reach your lowest or nadir PSA 18 months later which would be October 2026 - so there's a chance it still could go down.

    I only have his initial PSA and latest PSA to go on so:

    * Do you have a full list of dates and results for all his PSA tests?

    * Do you know when his next test is?

  • Undetectable is les than 0.1

    The 1.9 is not in itself good or bad but you will really need to wait for at least two more results to find the trend.

    If the trend is down - happy days.

    the other thing to remember this prostate cancer is slow growing.

    Doctors won’t promise anything but the likelihood was that the first psa would be less than 1.0 but I don’t think you should worry at all.

  • Hiya - I only have his pre- treatment psa test of 5.5 & the one he has had just now at 1.9. They have not said anything about when the next test is. I think its supposed to be every 6 months. Do you think we should get in touch with his consultant ? Thanks

    Yes, if those are the only 2 results you have I would get in touch with his team, express your concern and ask for 3 monthly testing.

  • Ok, similar to my stats really.My PSA  a bit higher at 13 , tumour size about 1.3 cm.

    More than one tumour but largest nearly on the gland edge.

    6 months after RT finished PSA 1.33 and been going down since then over nearly 10 years although my last PSA was one year ago.

    So too early to speculate with your OH but as long as a downward trend that is the main thing.

    So next PSA needs to be lower or the same although at some point there could be a bounce back temporarily.

  • Thank you - my Mum is on end of life care at the moment so I was starting to get really worried about hubby ! Not sure I can cope with anymore bad news right now. Hoping it continues to go down - will get husband to suggest to them another test in 3 months. Thanks. Ellen

  • Hello Ellen  At my cancer centre ‘undetectable’ PSA is classed as less than 0.1 ng/mL so 1.9 does seem quite high after you were told that. Particularly after RT and HT, where the level should drop to a low level very soon after treatment finishes, unlike RT-only, where it takes many months to get down to its lowest level. To put your mind at rest I think a conversation with your team would be a good idea at this stage. As others have said, trend is important, so arranging another PSA test at 3 months would probably be a good thing.

    If I’m reading it right, T3 puts him in CPG 4 and although the NICE guidelines say 6 months of HT before, during or after RT, up to 3 years total should have been considered. Maybe another question to ask his oncologist?
