DS1500 form query

Hi, I’ve requested a reassessment for PIP via report a change in circumstances option. I already receive daily living standard rate due to a long standing disability. Whilst I await  the assessment form I was wordering  whether anyone had been awarded high rate csre and/or mobility using DS1500 form and not having 6 months or less to live prognosis?  I’m on palliative chemo with a prognosis of 0-5% chance of living 1-5 years.  Older posts I’ve searched for are mainly some years old. Thanks

    You two are well respected big names on here who give great advice and support. When people on here don't know what to do they soon realise they have a couple of QCs on hand to sort them out.

    People looking after others interest always neglect themselves. Its like plumbers, they always have leaking taps!

    As I understand it if you have this form then you should automatically receive the highest rate of care.

  • Thanks @Tinkabel, I’m hoping so but the DWP telephone agent told me that a decision  will be made based on the info provided on the form re. eligibility. Fingers crossed Jayne

    Hi

    I received PIP higher rate mobility and care, the MacMillan Welfare rights worker did it for me without me having to do anything.

    I  am in palliative care with symptom control only.

  • Hi Tinkabel, I didn't even think about asking for a DS1500 Form, even after I had heard about it because I was told an approximate time for me was between 5-7 months, so not under the 6 months the you need for the form!

  • Just to let everyone know, I hags been awarded high rate care and mobility components of PIP!  Thanks for all your help and advice. Jayne

    Excellent & glad to hear it!!! Been claiming mine for 9 months now so getting my money's worth from all of that tax & NI over the decades...
