In my last blood test (in 2019) TSH and Tg values were 0.019 and 0.040 ng/mL. Now they have increased to 5.42 and 0.225 ng/mL. Should I worry about this new result?

No you shouldn’t worry but you should get your doctor to review your prescription and check your dose as it’s quite possible that you’re not taking enough thyroxine to keep your TSH at a suppression level. The increase in Tg may well be as a result of your TSH not being lower since the level you describe suggests that the TSH is stimulating the production of more thyroglobulin.
I’m going to take a guess that you’re possibly not in the UK as (quick google search tells me) TSH units in the UK arm U/L and I don’t know the conversion of that from ng/mL So I can’t really interpret your TSH change but the units for Tg are the same and the stimulation effect should be the same.
What you shouldn’t fear is that increased Tg means the cancer is back. Once you get the TSH stabilised, fresh Tg tests will tell you if it’s really gone up or not.
