BCC forehead nerve invasion pT3

Hello this is my first post. I have had a leison on my forehead for about 4-5 years and past GP's have given antibiotic cream to help it but it never healed. In December I was sent by a new GP for 2ww to dermatology and had a biopsy which came back benign. I had a dermatologist appointment 4 weeks later and thats where I was told it was BCC and I would need a wide excision. I had that 3 weeks later. On Wednesday I got my results and was told it had all been removed but I had focal perineural invasion is noted pt3 nerve diameter 0.1mm bcc. My consultant said the treatment would be radiotherapy but because of my age (44) they will not do this and I will have to just keep an eye on it. That has made me worry that it has progressed and I have to just leave it. Now I'm very poorly at the moment and my excision wound is infected, I have a massive coldsore, fatigue is horrible and have bad headaches. I know this might be my body just needing rest but also scared its to do with my aggressive BCC.

    I'm sorry to read how poorly you are and that your excision site is infected. I presume that you have contacted your hospital team and told them about this.

    As you are worried that this description, coupled with how you are feeling, means that your BCC is aggressive and spread, I think it's important that you arrange to speak to either your SCNS (skin cancer nurse specialist), if you have one, or your consultant to find out exactly what this means.

  • I think you need to see Consultant again. Doesn’t seem very clear to me what it means.  The questions I would be asking is how do you keep an eye it? What are the signs. Why can’t it be removed with MOHS microscopic surgery.  You could even pay to see a private consultant, you could ask him to explain why it seems acceptable to leave it. It costs about £200 to £250 to see one.  I paid to see one for my Infiltrative bcc in nose. as I was told I would be waiting 2 years.  The private consultant recommended to have treatment under NHS, but agreed it should be done sooner.  I have now had MOHS and a forehead flap and am having plastic surgery this week. I think you are stressed out and scared as I have been.  Your infection, cold sore, and fatigue will make you think the worse.  At the very least you need a thorough explanation of why it is safe to watch and wait.   The mental stress is awful, so if you don’t feel reassured after speaking to your consultant, seek a second opinion.  You need to understand your options before you can decide what to do.  I know exactly how you feel and wish you well.
