Have been recently been diagnosed with Advanced prostate cancer .Gleason 8 with both Lymph and one bone met in the pelvis following a PET ct scan.Both Bone scan and MRI came back clear.
I was diagnosed with locally advanced prostate cancer in 2017 had radiotherapy to prostate seminal vascual sack and one lymph node in 2018 then 3 years Hormone therapy 3 monthly Lupron shots till 2021 my PSA was undetectable up until 2023 then has slowly risen to 3.25 recently .My Oncologist was initially monitoring the rise 6 monthly but over the last 1 yr every 3 months .
I was shocked to find that following a recent PET scan that the cancer had spread to 2 lymph nodes in my neck ,2 in my abdomen and 2/3 in my pelvis with one bone met
I have no pain whatsoever and still don't .
I have been offered a 10 week cycle decotaxel but hormone therapy has not been discussed.
Is this normal practice???
Can someone please advise as I have requested another appointment with my oncologist to discuss the initial treatment plan .
The initial treatment planning meeting consisted of myself ,my wife and my oncologists registrar who was not aware of me being placed on 3 yr lupron shots when I was initially diagnosed form 2018 to 2021.
Please help any advice would be welcome .

Hi Paulo (Sharp E ), welcome to the club. I am not medically trained but my initial thought is that maybe they want to hit you with a systemic treatment to see the reaction first, as they know you respond well to HT and are perhaps saving that for another day? Definitely worth trying to get the consultants confirmation though. Please let us know how you get on. David

A warm welcome to the Macmillan Online Prostate Community - although I am so sorry to find you here. I am Brian, one of the Community Champions here at Macmillan.
From being around the Community for the last few years my understanding of your oncologist's thinking is:
* The Hormone Therapy has been used to reduce your testosterone and thus starve the cancer of it's food. This has worked up to 2021 (the 3 years). You have been off treatment for 4 years, your testosterone has come back and the cancer has spread.
* The Chemotherapy will be used to kill off the cancer around the body and then futher Hormone Therapy may well be considered.
I am at a loss as to why your team are not aware of your previous medical history and Hormone therapy.
Don't forget this is only my humble opinion as none of us here are medically trained.

Thank you Brian.They were aware as they have the notes.I just had to point it out to the registrar of my previous history and he had to look back through the note which I found a little embarrassing .My oncologist knows me well so I have arranged to see him Monday and I am hoping to gain an understanding of his thinking then.
