I have had the Dre mri and bone scan results today has anybody had a PSA this high or higher and not had an aggressive cancer that had not metastasised I look at those with elevated 5 s and wonder why my PSA IS 41 ! And hello

My PSA was lower than yours at 18 ish but the PC had got as far as a lymph gland and seminal vesticules.
Guessing that as you have only just received the results you havnt had a discussion with the hospital about treatment options. At this point they will be able to answer your questions and will have formulated a plan.

Try not to worry too much about PSA, it's always the first result, but once you have all the others, you'll realise that its main value was to flag up that there's a problem: PSA is notoriously unreliable, and the level doesn't necessarily mean much.
As time goes on, you'll see that rises and falls in PSA can tell you something, and the rate of rise or fall can tell even more. But one PSA result doesn't say much.

Hi just been given my result - 5 years terminal + hospice -Gleason score is 5 + 4 = 9 and 3b stage has escaped prostate - spread to lymph nodes and spine - will do hormone till ineffective then chemo and radiation then hospice then induced coma - I’m an ex soldier and it’s come as quite a shock especially for my wife - thank you for your kind replies I sincerely hope your outcomes are good kindest Raoul -T3B N1 M 1

Hi with the new medications and treatments, you can add on a few more years at least, if it’s not broke out of the prostate, then things are even better, wait till you get all your scans etc in, then see what treatments are good for you, if your just starting on this journey, you have a long way to go yet, plus as I’ve said hopefully many years.
I know your wife will be frightened, mine still is after five years, it’s just being positive, your here so keep it in your head never give in. My wife sends her regards, she knows what it’s like living with this thing good days and bad days, just remember she’s there for you.

Not good news at all unfortunately is how the surgeon began
I have inoperable cancer Gleason 5+4 = 9 and grade 3b which has got out of the prostate capsule - through the lymph nodes and has spread ( metasomatised) to the bloodstream and my spine - I will initially be treated with a chemical castration which will stop my testes and adrenal gland which also produces testosterone when not making adrenalin from both producing testosterone starving the prostate cancer of food - however this will only work for a limited time until the prostate cancer that has spread learns where the bone marrow is - it will then use this as a food store and eventually will deprive my body of oxygen rich red blood cells so I will then require new red blood cells from a transfusion this however cannot go on indefinitely and the consultant says when it approaches needing two pints a day it will then be time to be placed into an induced coma with pain killers and then unfortunately it is palliative care and regrettably time to say goodbye ! - I hope this has not upset anyone reading this but I feel that knowing the course of events once the cancer has escaped past the lymph nodes and spread to the blood and bone is one that you need to prepare for - I hope this helps someone - kindest Raoul and wife !
I'm sorry to hear your news, but remember that we are all unique, and no-one can predict the future. It may follow the path you've been told, but it also may not. Be sure you are included in all the decisions as things progress.

Hello Sorry to hear that you have been given such a disturbing prognosis. You didnt say if you were in the UK under the NHS system. For a consultant to be so sure of your Prostate Cancers path is unusual to say the least as men with Prostate cancer as aggressive as yours can be treated and there are new drugs and treatments being trialed all the time you may be suitable for a trial which is why I asked if you were in the UK . .It may not be curable but it is treatable and some men live for many years .I had a prostectomy in February 2020 it did not clear all the disease some had gone to localised lymph nodes and possibly some nodes slightly outside the pelvic area my Gleason is 4 3 (with some 5 ). .I was given Enzalutimde a wonder drug along with the standard ADT and had RT six months later .My base psa was 28 after the prostectomy but after 4 weeks on Enzalutimde it had dropped to undetectable and still is undetectable one year later .Having a prostectomy removed the mothership so my cancer load is lower and hopefully the RT has sorted it out .If you are your own advocate and find out all you can about any new trials and treatments and push to be considered for them with a positive mindset you may find the rather gloomy prognosis you have been given is not the one you end up with .I wish you well and hope the treatment you have gives you many more years of good quality life. .
Hi Rmaher, my wife and I have read your diagnosis, my wife was quite upset for you. I am 73 advanced prostate cancer which has spread to my hips, pelvis, spine,right arm, lymph nodes, ureter,and bilateral seminal vesicle, not that far from your diagnosis.
I'm on Decapeptes injections 6 monthly, Enzlutamide tablets, plus tamsolosin and Finasteride to help me pee.
My oncologist is very upbeat about my prognosis, we whish your oncologist was as upbeat, our best wishes to you and your family.
