Exercise/Cycling and Doublet HT

While my >2 years on decapeptyl (with EBRT) at time of original treatment 2021-2023 caused me few problems with "strength and fitness", that plus darolutamide for spread, recurrent, PCa, is now a different matter.

Second 3-mthly decapeptyl injection 19th Jan '26.and now that well into that plus the darolutamide, side effects becoming more noticeable. Less "energy", "stamina", whatever the term  -  received wisdom says 20-25% loss of strength and that corresponds with my cycling times/distances. Also cycling related, muscle ache in upper arms when on a bike where set up throws weight onto my arms. Swopping cockpits to raise front end on most used bikes!

However, difficult to separate out cancer and COPD effects! Oh, and age....

Anyone else found that HT second time round (especiallywhen elevating up to doublet treatment) greatly increases side effects? Any other cyclists with same experience?

Maybe just down to COPD and age!

Thanks, Dave.

  • I am on my first time round of HT, but can say that for me double HT ( Abiraterone with Prostap) has significantly more impact on stamina than single HT (Orgovyx alone) . I was on double HT for 2 years and my reduction in cycling stamina was greater than 50%. My weekend long ride used to be 70-100 miles, I could barely manage 30 when on double HT.  For the last 2 months I moved to single HT, and can really feel the stamina coming back, still nothing close to where it used to be pre-treatment, but a marked improvement over double HT.

    Hope this helps.

  • Thank you, G.

Clearly you started from a much fitter base point than I,  but then I am significantly older at 77, with COPD!

My longest rides last year were a few at 30-35 miles, simple persistence  -  I rarely miss a day  -  got me to just over 5,000 for the year. Right now I'm targeting min 70 per week!

So far no mention of other than double HT for life  -  not long then!

I still believe that as much exercise as the average over-75 y.o. without PCa can only help in combating tiredness and fatigue.

My best wishes for your continued improvement.

    Good luck, Dave.
