Psa rise?

Hi my mame is Jovan 77year.Diagnosis:2022y Gleason score 3+4 (ISUP Grade Group 2), clinical stage T2b
Pre-radiotherapy PSA: ~8.8 ng/mL
1 hormonal injection (androgen deprivation therapy) before radiotherapy
Chart with upwards trend PSA course after radiotherapy:finish 24.4.2023
2 years after radiotherapy: PSA = 0.2 ng/mL
3 years after radiotherapy: PSA = 0.38 ng/mL
PSA doubling time (PSADT): ~10–12 months

What can be expected next?

    Sorry to see you here, but you are in the right place to get tons of information.  I am replying to keep your post at the top of the new posts.  

    I am also facing more treatment after my PSA has gradually risen to above 0.2.  My next step is another PET CT Scan on 22/05/26.  The likely next step depends on if they find a tumour.  I do know the likely treatment, if they find nothing, is hormones and radiotherapy.  As you are in a slightly different place, that may be different for you - hopefully someone else will reply with a better informed answer.

    A warm welcome to the online Community although I am sorry to find you joining us.

    I am making a couple of assumptions here, do I assume you are outside the UK and you have been on yearly PSA tests?

    The normal procedure in the UK is for a clinical review after 3 rises or a rise of +2 whole points. Whilst I agree you have a doubling time of 1 year I think you will need at least 1 more PSA test before any further treatment reviews are made.

    I think with the PSA rises being "small" but a trend your team will be considering if it's a biochemical recurrence or PSA "bounce" after Radiotherapy. 

    if the rises continue the best course of action would be a PSMA-PET scan to see where the cancer is.
