Histopathology on TURP chippings - worse case scenario

Hi all

I guess by now it has become apparent that I am a worrier/over thinker. LH's histopathology report is showing on the hospital portal but is pending for 2 weeks. I know this is standard for them. He won't phone because he says it is what it is and they will contact him if necessary.  But of course my mind is in overdrive!

si realistically, what is the worse that it can show? He was a grade 9 from biopsy (4 +5). Can he go to a 10 from these results or is the result calculated as an average of this and the biopsy?  Can it show anything else bad; he already has cribriform and perineural invasion? Would any of this affect his treatment pathway? 

Sorry but I am the sort who has to know worse case scenario unfortunately.  Many thanks.

    Worrying over your husband is natural, you are quite right to do so. 

    Yes, he can get "promoted" to a Gleason 10 (I got "upgraded" from a 7 to a 9 with my chips) and the analysis of the chips can tell if there are any new conditions - they are checked for any changes in the cell formations. 

    To be honest, he's already a Gleason 9 with cribriform and PIN so I would think whatever the chips show his treatment pathway will remain the same.

  • Thanks Brian. What you say about the treatment is reassuring. I am still a bit confused about the grading. On the Dr Shultz video I watched he said that Gleason 10 is pure 5 with no other grade atJoyll. So would the chippings overule the biopsy that showed some 3 and lots of 4? Sorry to be a pain. As my mom used to say I need to know the ins and outs of a ducks a*** Joy xx

    So at present he's a "low" Gleason 9 (4+5) - I am a high Gleason 9 (5+4). IF he became a 10 it would be (5+5). A link showing the staging and grading is here:

    The "chips" would overrule the biopsy as they show the latest state of the cancer.

    Knowledge is Power and it's good to understand what is going on.

  • Thanks again Brian. So the cancer can progress to a higher Gleason stage even when receiving HT? That is bit worrying.....

    No it's fine really if you think about it.

    The biopsy was on 27 July 2025 - it was already an aggressive Gleason 9 (4+5) then and it depends where and how many cores they took. Don't forget he didn't start on HT until later. There would be a few weeks between the biopsy and the start of HT.

    The other point is the "chips" are bigger than the biopsy samples and come from across the bottom of the prostate so for the new grading they have a larger sample so any grading should be more accurate.
