Active Surveillance-A journey to where?

I’m currently on active surveillance. The details are in my profile with more information in my blog with the same title as this.

After initial worry of being diagnosed and spending hours on research about treatment and outcomes etc I became a strong believer in Active Surveillance if it is at all possible.

AS for me is a data driven strategy to reward patience with the gift of more time without treatment or their side effects.

So far my patience is being rewarded. Good luck to everyone on AS.  Remember AS is an active choice, not a passive one.

i will keep updating this post as time passes .

Oct 2025 PSA 13.2

  • Hi John

Always interesting for me hearing about experiences on a/s.

I’ve been on A/S for 4 years now with psa readings of between 6 and 10, but my last one was 10.6 and one before 11.3.

Consultant told me if two consecutive tests were over 10 I would need follow up so that’s where I’m at at the moment.

I Have annual mri in April so when consultant gets back to me I’m expecting for him to tell me to wait for mri results.

Im glad I chose a/s as given me 4 years of  ad normal life as possible.

As with everything though its a personal choice as to what treatment to choose

Also see your latest psa has come down so that’s a good signThumbsup

    All the best Roy

  • Hello

I have to say that I am not a fan of AS - I have lost a very good friend on AS and another one who went from AS to Stage 4 in 3 months.

Yes it may well be ideal for you but all I can add is to anyone is be sure you are happy to be on AS - it's cancer and it's not going away.

    Best wishes - Brian.

  • Hi John and Roy

    Thanks for sharing your experiences.

I've been on AS for over 9 years and I intend to keep it that way for as long as possible.

    More in my bio.

    Best wishes

    David

  • You mention PSA, but not tumour size.

It’s a good result to see PSA drop from 13.2 to 10.5. I wasn’t expecting that much of a change up or down. So I will have wait and see if I just have big swings.

I think tumour growth will be the most likely change to put an end to my AS. Currently I have two tumours 11mm and 13mm. Trying to get data on median tumour growth so I can predict forward has been patchy. I am quite relaxed if my PSA increases, but I’m predicting it’s still 18 months until PSA exceeds 20. If there is also little change in the tumour size.

I am using the only data I found for tumour growth. It used an annual volume growth factor of 1.23. With that both tumours should remain smaller than 15mm and clear of the prostate edge until the next MRI. When I can reassess.

Best of luck going forward. Let us us know how it goes with your medical team.
