Tadalafil 5mg

Same situation for me. Had the operation in April 2025. Been on tadlafil since. Nothing much doing up to now but had radiotherapy this year in March which had me on hormone treatment. 

A double whammy on the ED front 

Unfortunately they did nothing for me 

I am not medically trained but I think some consultants give them the non-nerve sparing patients as it is thought they stimulate blood flow in the penis.  I could be wrong.

You are basically correct, it does require these nerves to be effective.

I just typed, and recommend that you type into google/AI "how does tadalafil work after a prostatectomy with non nerve sparing surgery" and it gives you the full picture, including alternatives.

For those, like me, after radiation and HT it's different. My daily 5mg has stopped shrinkage to button size - very helpful when peeing. It also helps to stop penile atrophy, which could be useful one day when I finish HT in 18 months - fingers crossed!

I had great success with 10 or 20mg tadalafil before treatment. Just hoping I can get back to this. Meanwhile I get very occasional weak spontaneous erections (green shoots?) I also use a vacuum pump once a week also to prevent atrophy. Libido is very low, so just having to wait for that wonderful testosterone to 'raise its lovely head' again!

Do I read from above correctly that you are on Tadalafil 10 or 20mg? Up to now I have only been offered 5mg which do seem to be having some limited effect in the ED department and certainly have corrected the atrophy.
Maybe I should ask for an increase in the strength of my prescription at the next renewal?

Pleased to hear that you are having some limited effect on 5mg, I would take that as a good sign, especially as some important nerves may not have been 'spared,

I am currently prescribed 5mg. My 10-20mg was on private prescription pre diagnosis and treatment. My experience then was the (very good) effect lasted for 3 days, 

Based on this, I intend to skip a few days each week and stockpile these for 'special occasions' or holidays. I don't expect it will give the same effect as previously, but it's worth trying...

By the way, this week, to my surprise, I managed an erection after vacuum pump use. Good sign. Just need to find a libido pill now ;-)

Hi David,

Thanks for your thoughts and experiences.
My treatment was RT so there 'should' be no nerve damage.
I did read up on the 10mg 20mg versions yesterday and see that they are intended to be used as a 'per occasion' treatment where the 5mg strength is intended to be one every day which is what I am taking.
My libido has returned and the ED is only partial now it's the duration of an erection that is my problem.

Sorry Osca, mixed you up with Loobjo. Congrats on the libido comeback (3 years on) I'll have to wait....Maybe now If you try the 20mg you should 'rise to the occasion'
