Darolutamide vs Abiraterone for reduced Side Effects and QoL - Could the former be a better way to go?

Hi Again,

After more research, the clinical benefits of Daro vs Abi (AAT) with Hormone therapy (ADT) seem highly beneficial. It seems that the side effect profile is better on the fatigue front and that you also avoid the steroid (Prednisolone) side effects/withdrawal complications.

Does anyone have any experience with Daro SEs? Abi has just moved into 2nd place on my wish list.... I will have this discussion with my Oncy tomorrow re eligibility. Last time I  requested it, he refused, "Only for Metastic not Locally Advanced", but things change and it's worth a try!

  • Hi, can't compare from personal experience but from what I've read yes, daro "better". I started decapeptyl injections on 30 Oct last and darolutamide tabs on 1 Dec. For Stage 4 advsnced metastatic, lymphs "only", recurrence. 

    SEs are clearly, for all treatments, very individual. I had few for original EBRT plus >2 years decapeptyl. Now I've added darolutamide much greater effect on my strength, stamina, etc. My breathlessness certainly worse than 3 months ago, but that might be my COPD.

    NHS rules for darolutamide (at 4 grand a month!) changed in October I believe so yes worth asking. Cost is surely a consideration, linked to age and comorbidities.

    I have had 3 Leuprorelin injections and 6 weeks worth of Darolutamide so far. Side effects have been reasonably kind to me so far with only mild fatigue which walks off. Adding the Darolutamide has made no difference to my side effects in fact I feel better if anything.

    My initial diagnosis was PSA 25 with mets to one lymph node and pelvic bones. The first PSA after just Leuprorelin was 0.9 and was down to 0.16 after adding in the Darolutamide for 20 days at that point. I have also noticed and improvement in urinary flow and get up less in the night and the constipation that has plagued me for about 8 months is gradually disappearing too.

    For me although I would rather not be taking either of these drugs they are certainly working well for me at the moment.

    So pleased to hear your minimal side effects story...'"the Holy Grail"..

    I'm convinced that Daro is the best ARSI on the market (perhaps hence the cost of £4,000 per month and stricly reserved for M1 and castrate resistant.) So no Daro for me...just T3aN1Mo and I am not wishing that I'm not in theM1 boat (yet?)

    I start on the second best option, Abiraterone/Predisolone next week, let's see how the SEs go (mixed reviews, as usual)
