I was diagnosed with prostate cancer over nine years ago at the age of 55 with a Gleason score of 6 and have been managed on an active surveillance programme since that time. I have undergone two sets of biopsies and one MRI scan, the latter of which was completed approximately six years ago. Currently, I have PSA blood tests every three to six months, depending on the results. My PSA levels have remained relatively stable and are generally just below 5, which is reassuring. As I am now approaching the ten-year mark since my diagnosis, I would like to ask whether it would be appropriate to consider a repeat MRI to reassess my current status. Given the length of time since the previous imaging, I feel it may be helpful to ensure that there have been no significant changes. I would appreciate any advice on this and whether further imaging should form part of my ongoing surveillance plan.

So the NICE guidelines for AS say after year one a PSA test every 6 months and a DRE every 12 months - the MRI is only offered in the first 18 months - NICE guidelines here:

Personally I would be asking for another MRI after such a long time - however if you get one - well that's in the hands of the NHS. I think you have done well to get 9 years out of AS (Regulars on the Community will know personally I am not a fan of AS).

I'm around the same age as you. I've been on active surveillance for 9 years with Gleason 6. I have a PSA test every 6 months and MRI every 2 years.
I only had one biopsy. It was a bad experience and I refused to have any more.
My PSA is around 8, which is approximately double what it was 10 years ago.
If I was you I'd have another MRI. If you can't get one on the NHS you can do so privately by self-referral for about £500.
NG131 table 2 has details on managing active surveillance. There is no mention of a timeline for repeat MRI after the first repeat 12-18 months. There is mention of regular DRE.
All the way through the information “shared decision making “ is highlighted. I guess you could try the 10 year anniversary has brought the idea cancer has been growing for 10yrs and is playing on your mind. MRI would provide peace of mind.
