Hello All,
My OH (64) is stage 4 since last December, has completed chemotherapy as part of the triplet therapy and is now on 3 monthly Zoladex and daily dose of Darolutamide. In good general health, at work as normal, and taking regular exercise.
He had a phone appointment with the SACT nurse yesterday, who has sent him into a spin (he suffers with anxiety and depression) with the news that though his bloods are fine, a PSA reading wasn't taken, and he should wait until the next blood test, in January.
After all the close care he has received recently, this seems very far away. Should he request another test, with PSA?
Also, the nurse talked about options 'for when the Darolutamide stops working' or when his side effects become too awkward - he already has sweats, fatigue, aching joints all the same as while on treatment. All of this, has been very frightening - we had no idea Darolutamide may cease to be effective.
What do you think? How long does darolutamide work for, what is a possible next option?

Hi Jess40, my partner has just finished chemo on 2nd September as part of triplet therapy and is also on Darolutamide. He is having the Darolutamide prescribed by the nurse consultant I think once every 2-3 months but has to do a blood test 2 days before to include Psa levels so these can be monitored. I have heard that Darolutamide should work for approximately 2-5 years but everyone is different. My partner also experiences similar side effects as your husband. I have read that other hormone therapy, more chemo or olaparib can be explored which is good.
Hi Jess40 (& Zhenya11935efb ), I think I would ask for a PSA blood test now just to calm me down. It surely should have been done and was mistakenly left off.
All HT seems to gradually loose its effectiveness as the cancer works out how to become resistant. In my case, Zoladex started to fail after about 5 years and so I had more RT on the new met. Once that had been blasted, my PSA indicates that Zoladex is still working on the remaining cancer after 8 years. We are all different, so putting times against this disease is near on impossible.

I finished docetaxel chemo as part of triplet therapy in September 2024 and have since been on HT and darolutamide. All my regular blood tests (including the one I had this morning) have included PSA and I think your OH should ask for a PSA test straight away to confirm all is well and put his and your minds at rest.
I was told by my consultant before I started treatment that the HT / darolutamide combination can work for many years and it's impossible to forecast how long for each individual. It is still a fairly recent treatment regime and it's likely to be many years before the true effectiveness will be known, so personally I think it is unwise to try to forecast how long it will actually last.The good news is, should it stop working, there are always other treatments available.
