Update on husband

Hi

Following my previous post in August when we got the results of CT scan and MRI scan for my husband showing PIRADS 5 and likely bone involvement in the pelvic bone plus 1 localised lymph node of 15mm. I thought I would give an update and also see if anyone had any experience of treatment of low volume/ oligometastatic cancer (at diagnosis)

We received the results via email (!!) after chasing up and asking for release of medical records. Turns out all results were back over 2 wks ago and discussed at MDT but my husband was not informed. 


Results: psa July 7.1

Biopsy: Adenocarinoma confirmed. Highest Gleason was 4+5 but some cores were 4+4

9 out of 23 cores positive on biopsy.

Bone scan: High uptake in 2 areas in pelvis (Right and left iliac bone) 

1 small focal area on 1 rib - report says possibly a rib fracture (correlates with recent rugby injury to that exact area)

Rest of bone scan clear.

Staging T3a N1 M1b

So 2 possibly 3 areas of bone Mets. One possible pelvis (localised) lymph node shown on ct scan.

He has an nhs oncology appointment next week and we arranged to see a private oncologist too (I wonder whether a PSMA pet ct scan might be worthwhile and we are happy to get this done privately).

My thoughts are he may be offered triplet therapy ?  as he is only 48 and very fit/ well plus new nhs guidelines suggest also radiotherapy to prostate/ local lymph nodes is now being done for patients with 5 or less mets.

Also we’ve read about the new Stampede 2 trial and possible SABR directed to metastases as an option that is being trialled. 

If anyone has any experience of this or what treatment they were offered for low volume metastases I would be very grateful. Or anyone who is on the Stampede 2 trial.
