Just out a while from meeting the Consultant so will flesh out the results more later but here in a nut shell is what he said.
He started off saying they found low grade cancer in one sample. Took 12 samples, 2 from the tumour/lesion. Gleason score 6. Said the word Disconcordant results. He said it wasn’t what he was expecting based on the MRI which he thought didn’t look good.
Tumour/lesion at front on Prostate and said it’s tricky to get at. Said the MRI results don’t tally with the biopsy outcome in his opinion. Going to bring my case to Multi-disciplinary team next week to discuss the results and wants me in with my wife in 2 weeks to discuss the possibility of surgery or whether to go on active surveillance.
my thinking is maybe I need to do another biopsy as he mentioned the Glesason score that I got mighten be a true reflection of the disease in the prostate. Fact I’m just turned 50 he said we will have to do something and I don’t want to under treat it if possible. So will fill in more details later just getting my head around it. PSA level was 3.4 in May. So might get another PSA test done just to see. Any thoughts ladies and gentlemen? Thanks !!

PI RADS 4 is the high possibility of significant Prostate Cancer - yet you have a low PSA and Gleason 6 from the biopsy. Interesting!
My thoughts and don't forget I am not medically trained:
* I trust the MRI - something is there - you need another biopsy possibly under General Anesthetic to get a true Gleason Score and TNM.
* I am NOT a believer in Active Surveillance. I have seen people go from AS to incurable. I have never seen cancer just go away, so get it sorted as young as you are.
* Why oh why has he offered to remove it - that's major surgery when on the face of it it sounds like it could be treated with Radiotherapy alone - without Hormone therapy. There's also Brachytherapy (less invasive than surgery) which may be suitable.
* Yes it's 4 months since your last PSA - have one done but in the 48 hours before the test refrain from strenuous exercise and sexual activity.
Only my thoughts - others my think differently but I've set the ball rolling.

Super Brian, you have indeed set the ball rolling ! I think doing another Biopsy under general anesthetic might be the sensible decision. The Consultant didn’t seem convinced by the biopsy results at all and said twice that he was expecting worse results based on the MRI so that might be pointer that I’ve haven’t got the full picture. Surgery is not jumping at me as an option right now as you know yourself with the obvious side effects. Is this kind of result common enough where the Consultant doesn’t seem confident in the results of the biopsy Brian ?

I think some Consultants want every result to be pointing to Cancer before they decide to act! In my view the MRI is an accurate scan. A biopsy can miss the cancer and PSA is known to be inaccurate.
I have seen plenty of cases where the 3 results "don't match" and we are all different take me.
Original biopsy 29/12/21 - Gleason 7 (4+3).
TURP Operation biopsy of "chips" 04/11/21 - Gleason 9 (5+4).
Now that's some upgrade in 10 months!!
I would be pushing for another biopsy under GA.

Thanks Steve, yes the Consultant seemed genuinely puzzled too ! He showed me the image on the screen and pointed, see that dark area there, it should be lighter in shade like that white area at the bottom. I’ve requested a copy of the full results but still haven’t received them yet. With only 1 sample from the 12 showing a low grade cancer it’s better news than all 12 showing any grade of cancer but the fact he said twice to me that he was expecting a much worse result kinda leaves you in no man’s land as regards how accurate the results are. But let’s take the positives and hopefully it has been caught early enough ! Thanks Steve
