Treatment options

After a MDT meeting Greg’s bone scan was cancelled.  He had a phone call from urology nurse and said that the next stage is treatment options, which we have the appointment on Tuesday.  She mentioneTwo heartssurgery or hormone therapy and radiotherapy.  I want to go into the meeting prepared.  I have researched the treatments, but it’s a big decision.  The Gleason 6 combined with T3a makes me think therTwo heartss something more aggressive in there that’s been missed, but the nurse said because Greg was under GA, the biopsy would have been quite thorough.  Also, the lesion on one side is large 24mm and on tTwo hearts other side 9mm.  Greg is leaning towards HT and RT. I feel that surgery seems overkill being as it’s T3a and I don’t think I would believe it could be removed 100%.  I have started reading about brTwo heartshytherapy, but no one has mentioned this yet in the hospital.  It would be great to get everyone’s opinion on this.  Thank you Two hearts

  • Hi WW

I think Def RT and HT now , once T 3 gets more difficult.

But I remember your previous post, has it Def pushed through the gland edge, I thought it said gland edge bulging but sorry if Im wrong

. anyway it's good they cancelled the bone scan cos they probably don't think it's anywhere else.

    Best wishes

  • I have spoken to 3 different people over the T3a diagnosis.  One told me it’s because it’s so close to the edge, when I questioned if it’s broken through she just said she didn’t know but that’s the T3a diagnosis.  Another told me that the biopsy showed it’s bulging, but the biopsy showed the cells on the edge as suspicious, but not confirmed.  One said it has broken through because that’s what T3a says and it’s a ‘curve ball’.  His diagnosis has been given as T3a.  Because of the T3a it’s pushed the CPG to 4 and they seem to be talking about the CPG more than the other grades now.  It’s very confusing. If it is truly Gleason 6 i don’t feel it would have had the capabilities to break through, but maybe the size it’s just bulging.  I have read so much conflicting info, it’s driven me crazy.  I am leaning towards HT and RT too. Thank you for replying x

  • The reason I was going on about it was because I had a bulging gland edge but hadn't broken through.

It obviously isn't clear with his one but in a way ok cos if not then just like a T2, also good that a Gleeson 6 although that can break through, I was a G6 and suppose it would eventually have gone through.

Anyway sounds ok and fairly easy to treat and potentially curable

    Good luck

  • Hello

Thank you for the update - yes it's quite confusing, I am aware the best way to work out where a tumour is is through the MRI.

Brachytherapy may well be an option - it's possibly not been mentioned as the NHS Trust you are under doesn't offer it - you can request a review and a referral to the nearest NHS Trust that does offer it.

I am a T3a and also a Gleason 9 (I had a PSA of 182 - you can read my journey by clicking on my name or avatar) and had 3 years HT with RT to the Prostate and Pelvic Lymph Nodes. The only drawback are the HT side effects but I would do it again over surgery!!

    I hope the above helps.

    Best wishes - Brian.
