I was diagnosed with advanced metastatic castration resistant prostate cancer early 2023. Gleason score 9, stage 4 with metastases in some bones. No surgery but I've had radiotherapy and current treatment is 160mg Enzalutamide daily and Decapeptyl jabs every 3 months. PSA has come down to 0.78 so NHS Oncologist is pleased. The sole way of monitoring my progress is now the PSA level. But I'd like to investigate other options. Lutetium treatment is not (currently) available via the NHS, but some hospitals can give it privately. I've had a quote of around £15k from the Royal Marsden. Before I take things further I'd be interested to know if any other guys in similar situation have gone down this route and what the outcome was. I appreciate it may be early days abut any info would be useful. Thanks
My father had Lutetium-177 treatment at the London Clinic (paid for my health insurance) when all other treatment had stopped working it gave him extra healthy time with us, but sadly he died of a non related condition whilst that treatment was still working and available . He was 90 years old
Hi ChrisHodEx .
This is something we have discussed with the oncologist as hubby is working through the options following 3 recurrences to date. Traditionally it is given once Docetaxel has been through, but there has been a phase III trial with Docetaxel naive patients which indicates that it prolonged the time to recurrence by several months.
My husband has recently gone down the route of SBRT to deal with the met sites, leaving the Lutetium 177 for further down the line. We are also looking at immunotherapy, PARP inhibitors, gene therapy, further Docetaxel or Cabazitaxel.
I have done a search on the Macmillan site and have found several references to Lutetium 177 but little evidence of anyone who has actually had it.
This might help you decide whether you might be eligible for Lutetium 177.

I have also found a personal experience.

Not yet, but I have arranged a phone call Friday 28th June with a Dr Suh from Royal Marsden to discuss whether it's appropriate for me.
Thanks for responding Chris, would love to hear how you get on. I’ll also share any info I have after my dads appointment on 2nd July as he may be a candidate for it as part of a clinical trial.
best of luck with the call!
Hi Chris
My partner had Dr Suh for two second opinions, she's great,( I wish we could have her alll the time) I'm really interested in this thread and Lutetium-177. Is the 15k for the one treatment? I understand the concept of Lutetium-177 but not the process of how its administered.
Best wishes
L
Hi Lorraine.
If I can jump in here - my understanding is that it is given by IV over a short period. I have attached a detailed protocol if you want to wade through it.

I also think that is the cost per vial or single treatment in the UK but would like to be proved wrong.
Hi Ah,
Very interesting link and conversation. Thank you for that.
Lots of love
