Surgery or Radiotherapy? Decision made!

Decision made.  I’m pT3a, N0 M0 (see my profile for details ).  My extensive research points to a combo treatment of HT / RT / Brachy (NHS and NICE approved).  MDT met again a couple of days ago and their recommendation (without my input) was the same.  I had a consultation at Addenbrookes yesterday with a top prostatectomy surgeon and he agreed with me that my decision was the best one, as surgery would carry at least a 30% chance of recurrence.  So I collected my prescription on way out!   I took my first bicalutamide tablet taken this morning…..so the treatment is underway. Just over 62 days (NHS target) since GP referral so well done to them after a thorough diagnosis.  I feel like a weight has been lifted from my shoulders with regards to timelines (chasing the NHS, politely!), but I’m under no illusion that the next few months of treatment will present challenges.  My wife and I are a strong team and we intend to take everything in our stride and, hopefully, this curative pathway will prevail.  Here we go…..wish us luck! 

  • Brachytherapy isn’t suitable for everyone especially if you have bladder issues. Sounds like you’re in good hands. With warm wishes

    What good news - I am so pleased for you and again your information is brilliant for anyone on HT - and on the Brachytherapy pathway.

    I must say my Testosterone came back much faster when I finished HT last December, so we are all different.

    Good luck with your shopping - as things stand with me I may soon have a chandelier, wardrobe and batman costume for sale Frowning2 - it's already gathering dust!

  • AW, I was going for the natural way.  If it comes back naturally...

    I couldn't do 'regular exercise' without a magnifying glass!

    It was better when my libido wasn't there, because I never thought of what I couldn't do!  If you know what I mean!

  • Hi  - I take your point about “going for the natural way”, but my reasoning for using Tadalafil was purely to maintain blood flow to prevent penis atrophy. (It was not being used as a Viagra type stimulant). The dose was low (5mg a day).  

    Men have many more erections other than when engaging in sexual activity. The vast majority of these erections are during the night when sleeping.  These were the ones that I wanted to maintain.  A useful side effect was that I think it helped Mrs AW and me to also keep going with bedroom action (less often as before, because it required motivation: I had no interest and zero libido due to no testosterone).

    Now that my libido (and testosterone) has returned, my erections should return to be a normal reaction as they were before hormone therapy & radiotherapy, and I will cease using Tadalafil.  Hope this helps you & others to understand the usefulness of using this medication throughout HT.     AW

  • AW, I couldn't have Tadalafil because of all the other medication I was taking.  I've got so many pre-existing conditions.  I rattle when I walk!

  • Yes, I have heard that Tadalafil cannot be prescribed if there are contraindications.  Another way forward for you could be vacuum pump or similar?  AW

  • Another 3 weeks have passed. Just returned from another week leading clients, and busy catching up at home.  I had a full blood check yesterday, including PSA, testosterone and immune system (neutrophil and lymphocyte counts & ratio).  Just waiting for my NHS app to show me the test results… I’m a bit nervous TBH, as it’s 10 months since my last  HT Injection (3 monthly), so low PSA is no longer “masked” by testosterone suppression.  I’ll post the results on here when I get them.  Fingers crossed.  AW
