Hi. I am new here and would like to ask a few questions. I was diagnosed with stage 4 advanced prostate cancer.  1 spot on rib .1 on chest and 1 on spine. Gleason 8 psa 64.8. Just started my my lupron injection 3 weeks ago on March 23 2021. Doctor had me do full blood work april 14 2021. Doctor called today and told me my psa is 57. And my alkaline phosphate is 148 and testosterone is .8

My question is a psa of 57 after 3 weeks of treatment a normal drop or should it have been lower. Also he seems worried about the alk number of 148. And is the testosterone to low.

    You don't say what your initial PSA was, so I can't help on that one - PSA level on its own doesn't mean a lot. Some people start with treble figures, others with lower than 10. What matters is changes: obviously a drop is what is needed, and that's good. A rise rings alarm bells. But more important even than change is the Rate Of Change: a rapid rise would often indicate that current treatment is no longer working, while a rapid fall would generally indicate something good: it would always depend on context, of course.

    So you need to look at the level, the date of the test, and what was happening around that time. Along with at least two or three other PSA results.

    Yes, you're right. It is confusing. And that's the point. PSA is very unreliable marker, and really means very little on its own.

    The high alc phos might also tend to suggest that further investigations (liver / gall bladder?) are called for, but again, one isolated figure without context doesn't say a lot.

    I'd suggest you discuss these figures with your doctor or the specialist nurse: someone who has access to your notes and the full picture.
