Im new here. Where to go from here?

Hi, Im new here, so i'll start with some background. I'm male, early 40's from Yorkshire. Diagnosed with Kidney cancer in July 2024. Diagnosis was T1bM0N0 with clear margins, kidney removed via divinci robot in August. Initial scans highlighted 3 small lung nodules, 4mm, 4mm and 2mm in size, however had 111 referral in December 2023, for breathing issue. Thought it was Pulmonary Embolism, but nothing found on CT contrast scans or xray. Urology Consultant not concerned (as thought it could even be a benign kidney Oncocytoma), due to cancer containment within kidney.  Had full bloods taken at same time as scans which included a PSA test. Since found out I have early prostate cancer also, T2M0N0 prognosis. 2 weeks ago, I had a follow up CT scan, suggesting lung nodules had grown, but I've received 3 calls with 3 different sets of measurements, from local, area and regional hospitals. Volume doubling time (VDT) of nodules ranging from 3 months to 20 months, on largest one. With smallest, being recorded as no change, through to it's disappeared! I'm now being referred to regional hospital medical oncologist, for systemic treatment discussions and / or further monitoring (ct scan in Feb?). Whilst Urology Prostate consultant is deferring my prostate removal operation. He says I have time, to postpone this (1-2years), but lungs are the priority and that treatment (surgery) may impact systemic treatment, if these are now mets. I'm just struggling to get my head around, how this could be stage 4 now, from stage 1 (confident histology diagnosis) and now no chance of cure. I'm devastated. Has anyone been through similar with low stage Kidney Cancer being upgraded early on. Could these nodules still be benign? Thankyou in advance for support and for reading.

  • You are a complex case! Plus very unfortunate too having found you’ve early prostate cancer as well. I’ve had lung nodules and liver cysts with my stage four renal cancer but they don’t  change at all. Yours may be nothing serious and can take a while to find out. Im sure after stage one diagnosis yours are I’m on immunotherapy since radical surgery and it’s shrunk all my big metastasis in my abdominal cavity which appeared 8 months after surgery.

    you must be seeing your oncologist regularly so ask him a load of questions and don’t leave until you understand the answers. The body can hide lots of strange harmless things we wouldn’t know we’d got except we’ve had the scans! Try not to let your mind run away with bad thoughts - how often are you having scans? 

  • Hi Mistoff, thankyou kindly for your reply. Every 3 months now for lungs I'm assuming and due abdomen scan for kidney around same time as next one (feb), so i think they'll combine. I'll be under active surveillance on prostate, until they give go ahead to operate. Sorry to hear that you are stage 4. I hope the treatment works well for you going forwards. This is so overwhelming for us all. May I ask what initial staging you were given? 

  • I had no symptoms other than sudden weight loss and loss of appetite for about 8 weeks. Stage 3/4 initially told but after surgery staged as three. Had a violent reaction to first IV PEMBROLIZAMUB so it was stopped - a scan 5 months later showed huge mets had grown intra abdominally. I had surgery because I was in age limit, 73, but it was a very difficult procedure and I think cells spilled out uring surgery. This was on CT report. Your prostate will be ok waiting while things get sorted so don’t worry too much about that. My husband had a radical prostatectomy 9 yrs ago and is still clear so go for that if offered if you choose to. I have a strange attitude to my cancer- very accepting I suppose- something to do with being a theatre nurse most of my working life and having seen it a. But a little knowledge can be a harmful thing and I read too much when I can find relative papers and am quite fatalistic about myself. I’ve been fine with treatment really but it’s made my osteoahritis very much worse- or the time has-and I have a lot of leg pain and disability now which I never expected. Staging cannot be definite until specimens arrive in laboratory and under the microscope , before it’s a guess on CT reports. My 3 could have been curable but not now but who knows how long? I don’t need a bucket list!

    I know you are really hoping to hear from someone with the same experience, which I don't have, sorry.  Just wanted to say it must be such a shock to go from Stage 1 to what they are telling you now.  Especially as it sounds like the different hospitals have a different interpretation of the results.  I've no idea if they could be benign but I do hope that is the case for you.  Hope you get the clarity you need soon.
