HELP- Opt to try a Partial or go with full nephrectomy ?? Difficult location

my husband is seeking some help/guidance from others who may have gone through difficult decision process

Does anyone know of anyone who has it in a tricky spot within the kidney if so did you have total nephrectomy ?

medial border is unclear which makes positive margin in that area higher it is located almost in the hilum and is endopytic which makes surgical extirpation difficult

My husbands was picked up incidentally as most seem to be - It's Bosniak 4 T1 N0M0

It's been caught early which is awesome news and less than 7cm they believe will be slow growing however some evidence of his condition AS suggests that inflammation can have an affect in tumours and assist in their mestasizing which is a concern 

Normally due to size this would be a straight forward partial nephrectomy but due to the location unfortunately this poses many risks as they either won't be able to remove it all or they will try and will Nic main artery during surgery causing life to be at risk ending up in total nephrectomy anyway surgeon said he would be having heart palpitations doing a partial. (Obviously in an ideal world no surgeon would want to see a 40 year old loose a kidney if a partial was possible) but we are where we are in terms of location which can't be changed. They have said asking for full removal from outset is not unreasonable at all

my husband is most scared of them to try a partial due to risks involved he can have a biopsy for them to see if low or high grade but biopsy comes with risks itself (hitting main artery, bleeding post biopsy, seeding, inconclusive results)

He ideally just wants them to say it's high grade and taking the whole kidney so he doesn't have to make the decision
However if comes back low grade or inconclusive he then still doesn't have answer and has had another procedure and puts himself at other risks in process?

For me due to location I would opt for full nephrectomy but he wants others views on who have been through this which is where you lovely lot come in :)

Atleast if full removal It's all gone and he can stop worrying? His only 40!

would appreciate your views of those who have gone thought this difficult decision and how you have coped after 

Did you have a biopsy?

Did you have a partial , any complications after?

Did you have a full nephrectomy , any regrets ?

    Hi, my partner is 36 and is in a similar situation to your partner, he has to have the whole kidney out because of where the mass is, he has had 2 biopsys which came back inconclusive, and the consultant has said its his decision whether he has it removed.

  • Hello, i am in some ways in a similar position to your husband in that I have a difference in opinion to the way I should be treated. i was seen by an NHs doctor who thinks that I should have a partial. I have renal cell cancer on my right kidney and a bosniak cyst graded 2F ( meaning  it is going to be followed up).on my left.  Due to this the surgeon wants to keep some function in my right kidney in case the left goes a bit 'scooby'.

    Due to the long wait under the NHS I went to see someone privately and his.view was that the whole kidney needed to be removed. he wrote to the NHS consultant with his findings and his rationale for taking the whole kidney. I was then contacted by the NHs consultant who had received the letter said that his decision remained the same, just a partial.

    It is so hard to make these decisions ourselves and I do feel for you.

    I did have a biopsy which confirmed the diagnosis. However this will have delayed my nHS op by around 6 weeks, waiting for the biopsy to be done and then the results. 

    I presume that in your husbands case they are trying to keep some function due to his age.

    I have now booked in to have it done privately on April 28th and am having a radical nephrectomy.. The nhs were looking at mid to end of June at the earliest. I can only speak for myself but can say that I have felt so much relief in my mind knowing that I have a date and it's all coming out.

    Thanks so much for your reply Jules, my husband has also opted for full and op is 29th April Slight smile The surgeon said any choice was acceptable and up to my husband so long as his happy for total removal then obv the surgeon is happy as a partial would have been very hard and likely end up as total anyway. Where are you having yours? My husband is having at Princes Grace London

    Due to age yes ideally partial would be better but due to location it pOsper too many risks in our opinion and from hearing other peoples views / stories some who had partial wish they went full. We have caught it early so let's get rid of it all and pray to god that will be the end of it . Also radical surgery is more straight forward than partial so hopefully recovery better for him and you
