Pain Relief - Next Step

Hello again everyone. Radiotherapy us ongoing, 14th session completed today, 16 to go.

ihad a good discussion with the dieticians today and due to increasing swallowing difficulties and loss of taste, I will be using my feeding tube more.

The pain around my oral cavity has increased to the point where the paracetamol+Oramorph is just not alleviating the discomfort for more than 40 minutes; with 4 hours between doses. The Dieticians will speak with doctors to recommend the next level of longer-lasting relief; can anyone tell me what this would consist of?

  • Hi it’s normally MST a long lasting morphine I didn’t have it but many do. You take it at night snd it last through the night.

  • Good evening Bob, i remember after one of the biopsies the pain was so bad I had to have Sure-Tec( similar to Oramorph)and fentanyl pain patches, so the main painkiller was the pain patch and topped up with Sure-Tec. It did the job and was only on it for a very short period of time. I hope this helps. Yes, i would use the PEG as much as possible to give your mouth and swallowing a break, believe me, they are better going through the Peg than orally as most of them are soooooooo sweet. Sounds as if you are making steady progress and will be halfway after the next session, so well done. Take care.

  • Hi Bob. 

    I had MST which is long acting morphine that comes in sachets you can dissolve and get down the tube. It provides a good level of background pain relief which you top up through the day with oramorph. My oncologist told me I could take Oramorph every two hours if need be but that just sent me to sleep all day. If you are getting breakthrough pain that often do tell them....you might get Fentanyl patches which do the job much better than MST. Best of luck and let us know hoe you get on

  • A few hours after RT today, I had to call the support team as I had a raging sore throat and I had vomited a yellow mucus a couple of times. I was requested to attend the cancer care centre assessment where they did various tests. When the nurse shone her torch in to my mouth she was shocked to discover a bad infection of thrush. Blood tests have been taken and, unless something bad shows, I will be allowed home this evening.

    Wilma is at a family wedding up in Blair Atholl today, we decided weeks ago that I wouldn’t be able to attend, and when she called to ask how I was, it was difficult NOT letting her know where I was. Hopefully I will be home first.

  • Ah yes. That would be the cause of the sudden escalation. Fluconazole for a couple of weeks will sort it and you’ll feel better in a couple of days. Thrush,  the bane of our existence. I got it a couple of times, once at the end of treatment and again much later after a course of antibiotics. That’s something to be on top of in the years to come. If your on ABs get Fluconazole too. Hope you feel better soon

    Sorry to hear this, hopefully they are getting it sorted.

    I ended up with Frntanyl patches and paracetamol with Oramotph top up if needed. I was away with the fairies on all that though!

    Thankfully I avoided the thrush. I did get bad reflux though which made me sick, despite the Lansoperazol. I ended up with an antacid and oxetacine to take 30 mins before eating, then swill with difflam 15 mins before eating. That numbed me enough to have something. I did top up with PEG too. I also gargled but spat out dispersible aspirin to ease mouth pain. If once thrush sorted you still struggle might be worth asking if anybof those possible for you?

  • Hi Bob I ended up with a combination of MST and oremorph...to be honest I would have taken anything to ease the pain and that combination worked. It took a bit of getting off at the end but it worked for me  whatever you get remember it is only temporary and you will get off it eventually when the pain subsides and it will. It took me 6 months in total to get off all the medication and 30 months later I lead a fairly normal life . I found walking daily improved my fitness but also gave me time to process what I was going through. It does change your attitude to what is really important and I live a different life now only working part time and spending more time with my family and playing plenty of golf.
