What questions should I ask the oncologist?

Hi. My husband had a recent endoscopy after experiencing difficulty swallowing food. During the procedure they identified something suspicious and took biopsies. He was sent for an immediate CT scan. Last week, after the multi disciplinary meeting we were informed that he had a squamous cancer high in his oesophagus and also some “speckles” on his lungs. We were told that his options were limited as (on paper at least) he is not physically fit enough to cope with the chemotherapy. However, the nurse giving us the news thought that, having met face to face, that my husband was borderline and should be seen by an oncologist to make the decision. What questions should we be asking in the meeting, and how can we push for chemo if the oncologist doesn’t think it would be appropriate - given that the only other option seems to be palliative care!

    i am sorry to hear your husband has had this diagnosis.

    Given what you have been told my first question would be what is meant by speckles on the lung?
    I would also ask for staging in terms of T?N?M?
    Although it sounds as if your husband has metastatic disease from the news the nurse has already given him.  
    If you do not know already, I would find out why your husband is considered unfit for chemotherapy? Is it pre-existing conditions? Or is it because of the OC tumour burden? Will chemo make him feel more poorly than he feels now? (Explore quality of life vs. Quantity of life). 
    Enquire about other treatment options which although may not be curative might be supportive and relieve symptoms. His nurse has not mentioned radiotherapy do you know why that is?

    These are only a few suggestions.  I hope you will get more from others in this forum.

  • Hi there. I would agree with all of the suggestions that Rosie has made for when you see the Oncologist. From my own perspective, having been diagnosed with Squamous Cell Cancer in the upper part of my oesophagus in May 2023, I was told of the possible side effects of chemotherapy and radiotherapy such as neuropathy, diahorrea, nausea, fatigue, tiredness. Maybe knowing the side effects will prepare you to ask more questions. It is also pertinent to ask whether it would be worth trying the treatment, to see how your husband copes. I’m sorry I can’t give you more ideas and I hope that you both become better informed about the options available. Will be thinking about you. Best wishes, Julie
