Tamoxifen and Topical Vaginal Oestrogen

Hi

Can I please ask if anyone is taking topical vaginal oestrogen?  I met with my oncologist today and he mentioned it as a possibility as I am struggling with vaginal moisturisers giving me BV and I am always having to dash to have a wee when I don't really need one.  However it concerns me that I am taking tamoxifen to stop oestrogen so adding a topical oestrogen seems odd.  My oncologist says that the tamoxifen will take care of any oestrogen that seeps into the bloodstream but I'm torn and confused!  If anyone is taking topical oestrogen and can help I would be so grateful!  Thanks so much! 

  • P.s. I found a paper on vaginal moisturisers after breast cancer and it said that they frequency of use needs to be more than in the case of a natural menopause.  Every other day instead of every 3.  I changed to using it every other day and it made a massive difference toitseffectivenes.  X

    I started using Vagirux in January this year after almost a year on Tamoxifen. It's really helped me.

    I was anxious at first, but my oncologist, my surgeon and my GP all agreed It's safe to use. I also trust Liz O'Riordan's advice and she confirms it's okay too.

  • Yes this was an excellent study and the issue for me is I am on anastrazole and there is a risk of recurrence on vaginal estrogen treatment while taking AI’s.  My medical oncologist said no vaginal estrogen for me as my tumor  and DCIS were both 100% estrogen and progesterone positive.  I use a hyaluronic acid vaginal suppository every 3 days snd that helps me.  I am sure the vaginal estrogen would work better but I don’t have UTI so ok for now.

  • My tumour was ER +8 and PR +7.  After considering my nomogram results I decided not to have endocrine therapy.

    I did take ERT for 20 years, so do not match the selection criteria in that study.  I am still gathering information to consider whether the benefits outweigh the risks.  I have VA and persistently high leucocytes as well as a recent UTI, which indicated resistance to a number of antibiotics.  I, therefore, feel I need to be more proactive in my approach to the problems.

    I did note that statistically there was a potential risk of recurrence for those on AIs, but not on mortality.  Depending on how long you have been on the AI I think it is possible to switch to Tamoxifen, but there again it is a risk calculation.  I thought DCIS tumours were less threatening, ie stage 0.

  • Hi - I have been treated for hormone positive breast cancer and am on Tamoxifen. I am also using Estradiol pessaries which each health professional I have spoken to have said is now considered to be 'safe' as the oestrogen has a highly localised impact. I am not using a topical one but a tablet placed in my vagina 2 times a week. Seems to be working. Good luck.

    According to a 2022 Danish study you should be in a good position as you are taking Tamoxifen.  It is a SERM which should protect your breast tissue from the oestrogen should it pass into your system, during the loading period for example.

    I am in a different position because I am not taking endocrine therapy.
