High grade DCIS

Hi just joined and am really upset confused annoyed especially about my last appointment. So sorry this is long. 

Have family history of breast cancer mum at 65, my aunt at 38 and 2 cousins in their 30's. Was having yearly mammograms since my mum was diagnosed. Got to 50 they stopped. 

Then during pandemic june 2020 noticed a nipple change got seen within 2 weeks was told not to worry was microcalcifications sent away. Then in routine 3 yearly mammogram november 2021 had my mammogram I had a call 3  weeks later to tell me they had cancelled my appointment at King's. I did not know about the appointment and then an admin lady told me that I needed further investigations. 

That same day I called the breast unit and spoke to breast nurse. I went along to the appointment and they where great. They where kind and I felt relaxed and not rushed and they explained things. Due to Christmas and new year bank holidays there was a delay on the biopsy. 

Results showed in the same breast I had noticed the changes high grade DCIS and LCIS. They explained everything and my care was transfered to Guys. The plan was for a mastectomy and because of the strong family history they suggested that guys would consider a double mastectomy. 

We went away and I read all the information and waited for my appointment at Guys. All during this time I carried on working as a midwife as it kept me busy. Not sure why had not had any melt downs and felt practical about my decisions and always positive. Did not want diep as did not want any further surgery felt like a double mastectomy was fine as this meant no more worry due to family history and implants was the best option. 

The waiting had got to me and on Monday went to Guys with my partner was not expecting 5 people in the room. They made their introductions and then said what needed to happen. There was lots of different options all of them talking that at one point I had to say I was confused my partner and I looked at each other. During the appointment they even made a joke about putting on weight if I needed a second mastectomy if the geno sequencing comes back positive if I choose DIEP. 

We left the appointment with more questions we both felt rushed and what Kings had said would happen is the total opposite. I felt like we went in and basically the attitude  was we see this every day here you go this is the options off you go choose one call us back with you decision by Friday and surgery will 3 weeks later and gave me a macmillan book to read.

We left and talked on the train we both felt that the appointment was not great especially in not being treated like a person rather than the next on a conveabelt. So now my thoughts changed I am so worried now with my family history about at some point in the future getting BC in left breast. At 57 do not feel that having 1 implant is the right choice as I will need to look at myself daily and 1 breast will age and other will not. So now feel like I will have to go down the DIEP decision which I was anti as did not want any other surgery. Although have read some real positive things on here which has helped me and led me to post this on here. 

So sorry this is so long but I have not been given any contact at the hospital to call and can't call the breast nurse I was under before as she is from a different trust and no longer under them. 

Can anyone help give me some information feel let down

  • Five people in the room?! I would not have expected more than two -- consultant, nurse. On the one occasion my consultant wanted to include one of his registrars he asked me if I was ok with that.

    Your family history makes me feel you should be offered genetic testing. Where do things stand on that at the moment?

    What options have you been given to choose from?

    The answers to these questions would make it easier to point you to information that may be relevant to you, or at least suggest questions to put to your nurse with a request to have them answered by medical oncology or the surgeon. 

  • Hi thanks for the reply. I have been offered genetic testing but would not get the results for at least 10 to 12 weeks. Then if positive a double mastectomy. Or have a right sided mastectomy with either implant or using my own body tissue. Then once I get results if positive can opt for left sided mastectomy but may not have enough tissue. 

    Have called them today and have another appointment next week.  

  • Yes, the genetic testing does take a while. Do you have the option for delayed reconstruction? If so, you may want to consider waiting with that until you know whether or not you are going to want to have the other breast removed, and either way, I would bring this up when seeing the surgeon, just to see what the options would be in either case.

    As for the appointment with the oncologist, I find it very helpful to prepare a written list of questions ahead of time, then I go through them one by one during the appointment.
