New transitional cell carcinoma

Hello all. I’m a senior ENT surgeon in the UK, age 60 years generally fit and well. Had unexpected haematuria and a kind colleague did CT scan on me today. 2cm TCC bladder with no invasion on the scan. I’m having TURB tomorrow. I haven’t had a chance to discuss with the surgeon regarding further therapy but he did mention mitomycin single shot. I’m as ignorant in this field as anyone and would value any personal recommendations from experience. How long is recovery from this procedure as my managers would want me back ASAP due to very long ENT waiting lists!! Many thanks.

    Welcome to this group, but sorry to hear about your diagnosis.

    I was knocked out for my first TURBT and stayed overnight and was very well looked after and I'm sure you will be also.

    I was signed off work for two weeks, the doctor didn't give me any option on this front (just told me).

    I felt ok after a few day's but glad I had time off to fully recover. Although your work is incredibly important make sure you feel better.

    Sorry you have had to join this group, I'm sure none of us want to be here but the lovely folk here are full of good advice and plenty of support. 

    I had a cystoscopy in February and a CT scan the following day and was told that the tumour would be removed during a TURBT and thatbI would have a dose of Mitomycin and then just be monitored. From there the news just got worse and worse. My tumours x 2 were removed the largest was over 3cm and then the Histology result was that they were High grade urothelial papillary tumours.G3 pT1. So I needed a further TURBT 4 weeks after the first to confirm the diagnosis. Now started on BCG therapy.

    Please give yourself time to process this diagnosis. I also work in the NHS and am aware of how valuable Staff time is, but you are an individual and you have your own needs as well as those of your patients. When staff leave the NHS they are quickly replaced but you cannot be replaced in your family and friend circle.

  • Hi Hopeful, welcome to the group, I was 62 when my tumours were discovered, was shocked as very healthy up until then. Had them removed, stayed in overnight, rested for 2 weeks and went back in gradually, then another Turbt about 9 weeks later, diagnosed grade 3 PT1 and started on bcg November 2022. Been coping well with bcg, had mixed with 3 heated Mitomycin at the start, 2 clear cameras and one needing bit of investigation today. Been working throughout. This group is invaluable for learning and support. Good luck on your journey and put yourself first. 

  • Thanks so much for sharing your experience. My TURBT went well today. Had it done under spinal and heavy sedation. Main problem now is very painful urination as it goes through urethra, probably from cystoscope. Nice to hear that you’ve been working throughout. 

  • Drink lots of water and the burning will lessen. I found this to be the most difficult effect of the TURBT. It should improve significantly in the first couple of days (urethral burning) but I did not feel fully back to normal for a few months afterward. The fatigue likely will persist at least all of the first week.

    I'm so glad your TURBT went well today. After my first one I passed out in Hospital bathroom and came round to the Crash Team surrounding me asking if I'd had a cardiac arrest Laughing.

    Keep drinking lots of Robinsons Barley Water as recommended by Denby and take some time to recover, you will soon feel better.

  • Yes but I am administrator in NHS so no where near in such a stressful role as yours. Glad your Turbt went well, yes I remember it all being bit painful and feeling quite exhausted by it all but just rested up for the two weeks and then felt much better. It's all the stress of it all too isn't it. Have a well deserved break.
