TURBT + Nephrostomies

Hi,

I was wondering if anyone has had a similar experience or info about the following things.

My grandad had nephrostomies (bags that collect urine from the kidneys. The urine doesnt flow into the bladder) placed due to low kidney function. so he no longer urinates the "normal way".

He had a TURBT about 2 weeks ago.

For about 10 days he did excrete blood "the normal way" i guess thats expected from what i've read. however since then, there hasnt been any blood or wee "the normal way" and its just urine in the nephrostomies.

For the last few days hes experienced urgency to pee, but obviously doesnt pee because he has nephrostomies. but after this feeling of urgency he gets a lot a lot of pain in his lower bladder. and it lasts for about 20 minutes (This is the area they expect the cancer to be. We actually dont know if its bladder or prostate yet) 

Has anyone found a way to make the pain less painful?

He cant take ibuprofen due to his low kidney function and he reacts badly to morphine products including codeine. So hes only taking paracetamol.

Secondly, i read about how to look out for complications but they all relate to peeing, eg lots of blood in the urine, clots in the urine, pain when urinating, frequency etc etc

However if you dont urinate "the normal way" how do you know if any of that is happening? how do you know if theres a big clot or anything? How do you know if theres any issues in the bladder if nothing comes out of the bladder? you cant test for a urine infection or a bladder infection because there is no urine to test thats passed through the bladder.

Im worried we'll miss something important.

We have the number for a specialist nurse but they dont work on the weekends so weve had lots of questions building up. We will hopefully speak to them tomorrow.

  • Hi and welcome to he community. Passing blood and blood clots about 2 weeks after a TURBT is normal as the scab breaks away from the wound and we would normally advise lots of water to flush things out, but obviously with the nephrostomies, this is different. I have no experience with nephrostomies, so unable to advise. Hopefully someone will be along to help. Whichever, you need to call the nurse for advice. I hope all goes well. Keep us posted.   • Hi, Afraid like Rily I can't help about nephrostomies. But the bad reaction to morphine/codeine rang bells. Most NHS docs unfortunately are completely unaware of the following. About 7% of the population have dud genes for the liver enzymes, "CYP450" that process those painkillers, many antidepressants and some other things inc certain antibiotics I believe. This means you get the bad side effects and get less or no 'intended therapeutic benefit'. I found out about this because it affects Mr. D. He has Asperger's and about half of people with this or Autism have the gene problem. I wanted to let you know that there is a possibility that this is why your Grandad doesn't get on with those painkillers, because it sounds as though he will be in need of all sorts of medication going through treatment, and if you can help the doctors pick suitable meds it will help him.

    Thank you so much for this information. I will look into this further as my grandad has sensitivities to many medicines and this could be a potential reason why. I have a family member with Aspergers also so it will be good to keep this in mind for them aswell.

    Thought i'd update in case it can help anyone else. After speaking to the urologist, we were told the extreme pain was due to muscle spasms in the bladder. For this my grandfather has been given a muscle relaxant but it will take up to 2 weeks for it to have a proper effect. Theyre testing out a few different pain meds to cover the pain for now although its really hit and miss. So if there is a lot of pain its worth getting it checked asap as i guess the pain can be managed but it will take time!
