Need advice about stoma which I have had since January this year

when I was in the hospital for three weeks and had nurses around I never had an issue with my stoma bag leaking but since I had been at home it is happening every couple of weeks. Is it that I am not sticking the bag to my skin properly, does sweat make that adhesive less effective? It is bad enough that I have to wear this stoma bag but it could at least work properly....what do you suggest?

also are stoma bags waterproof for baths and swimming pools?

    Hi Ian, 

    following your advice I added skin barrier wipe, skin barrier cream, brava elastic tape and protective seals to my latest order as I had experienced some more leakage. So yesterday evening I added the protective seal which I placed before the stoma bag and then the elastic tape to hold the bag in place when there were noticeable bulging at the base of the bag. My husband helped me with it all and it seemed safe so I went to bed. I woke at 4am with poo everywhere. It seems the 'output' didn't travel down into the bag as it always does but seemed to have got stuck right around my stoma and it found the only way out which was up and to the side.

    i was devastated thinking the new efforts i had made would work.

    do you think I possibly ordered the wrong size? I ordered 34mm x 2.5mm.  My stomas are cut to 35. I don't see how else the poo was just stuck at the top.

    I obviously don't want to use one again, if I ordered the wrong size will Coloplast charter just exchange them for me?

    sorry to bother you with poo this early on a Saturday, your help would be so appreciated.

     To be honest I feel like I'm going backwards, I've been applying my own bags since I got out of hospital on 15th Feb this year. No problems for ages now it's happening more frequently, I don't think that I am doing anything differently but it is making me feel like I have completely forgotten how to attach the bags.

    Sorry to be a pain but I have not received a response to the above message.  Since writing it I have had yet another leakage, I will contact my stoma nurse about it today but I had really wanted your advice.

    Hi Jennylou

    Sorry for the delay, I'm in hospital myself and some days are better than others to answer depending on the daily medication changes.

    It sounds like you've experienced Pancaking.

    Pancaking is a kind of ‘vacuum’ where the inside of the pouch sticks to the stoma. This may lead to the output not dropping down into the pouch and building up on top of the stoma. Lubricating the inside of your pouch can help output to travel to the bottom of the pouch. See below for recommendations.

    Filter control

    Using the filter cover provided with your products can help minimise pancaking. View the video for more information.

    • Trapping air inside the pouch can help. Before applying the pouch, apply a filter cover and once the release liner is removed, blow into the pouch to insert a pocket of air. Finally, apply the pouch. Placing a cotton-wool ball or scrunched-up tissue into the pouch can also work to trap the air. 
    • If you are using a one-piece closed pouch and the above doesn’t help, consider trying a two-piece appliance, or an open pouch, and de-vacuum it through the coupling or open outlet on a regular basis, for example every time you pass urine.
    • Using braces or high-waisted trousers can also help reduce pressure.

    Basically ensure that the bag has some air in the bag by blowing into it before applying.

    PROTECTIVE SEAL

    There are two trains of thought on how these should be applied which creates a difference of opinion

    Many stoma care nurses advise that the seal should be applied to the bag before it is applied

    Many others, myself included, prefer to mould the seal round the stoma and fill in any irregularities and make a nice snug fitting of the seal, then apply the pouch to get a second nice snug around the stoma. As they are elasticated you could make them bigger and they contract to give the snug fit

    Unfortunately many of the things we try don't stop leaks all we can do is try to minimise them and get the one to three days the Ileostomy pouch will last by continually checking all round the wafer for tell tales that a leak is imminent plus the itching or burning even a burst of smell can be an indication.

    With pancaking normally all you need to do is give your bag a pat or shake the contents down.

    Jennylou, no you have not forgotten how to attach bags, you are experiencing your  trial and error syndrome where you keep having to tweak things until you find the ideal solution FOR YOU on how to manage your stoma and become the old you and your husband wants you to be.

    Sorry for the delay, hopefully they will sort me out soon and regulate the drugs and I can be back to normal.

    Ian, I am so sorry to hear that you are in hospital. I hope that they'll sort you out.  The last thing you need is me whining that you didn't reply. Thank you for your detailed message. I can't see how I can mould the protective seal when it is sticky on both sides.  I also don't recall getting a filter cover but I will watch the video.

    Don't worry about asking me questions, I'm just taking a little bit longer than usual to answer.

    Protective Seals normally come Circular shaped with a little hole in the middle they are made to stretch in all directions, and the I fit mine is  by

    Pulling in all directions to enlarge, the I place the new enlarged hole along the top of my stoma and gently stretch it until the whole of top is nice and snug, then I move to the sides and pull each side out then gently release and let the seal find a snug position against each edge, finally the bottom edge you will  has found its position and by checking with your mirror you will be able to check if there are any gaps or any you want to sort out you can lift the seal and move it as you want. Gently rub your finger around the seal to complete adhesion and you are ready to fit pouch.

    FILTER COVERS

    In every box of pouches you receive you will find a sheet of seal adhesive labels, these are the filter covers.

    I'm sure you doing well with your stoma management you're just have a bit of a wobble which we all experience, we never remember the good changes but the bad ones always stick in our mind.

    I have found the filter covers, not sure what to do with them but I do have them. 

    Regards the protective seal the hole seems to be too big for my stoma, certainly don't need to stretch it to make it bigger. Do they come in different sizes?

    very good idea about the bedding items, I will look into it.

    I've had a tough few days with a leak five days in a row. 

    My hubby suggests maybe I need a different stoma for what may be my more delicate skin. I said don't be daft as I have boxes to use and they won't take them back

    I call these things donuts and mine come from Dansac. They come in 3 sizes. An alternative way to fit them is to make a single cut so you can open up the seal and wrap it round the stoma. Means less moulding needed that way. Might be worth giving it a go...
