My husband died last Wednesday and a doctor from 111 came out the same night and verified the death. She told me that she would send the copy of this that evening and the GP's surgery would then forward it to the medical examiner who would then call me and forward the certificate to the registry office and then I could make an appointment to go and register the death and obtain the certificate. I cannot find out where we are up to with this procedure. On Friday I rang the GP surgery to ask if the had sent the form to the ME, she said no because we have not received the verification certificate from the doctor who verified the death. I was sat on my PC at the time with my husbands medical records on the NHS app on the screen. I said yes you have it is right there on his records, also the doctor had filled in a certificate and I wanted to know if it had gone to the ME. She said she still could not find it. Later Friday morning the GP surgery phoned me and asked me if I knew the registration number of the doc who came out to verify the death, I told her it is on the certificate you already had. She seemed confused and I gave it to her anyway. This morning I rang again to see if the form had gone off and when they sent it. She disappeared, came back and said that it had been sent to the ME who had sent it to the registry office. I asked her about the phone call I am supposed to have had off the ME but she said she did not know the procedure regarding that. I asked her when they sent it, again she did not know that either. I am no further forward and have relatives who want to visit the chapel of rest and I cannot get my husband into the Chapel of rest until I get the certificate, I am unable to make any funeral arrangements until I get it. Has anybody any suggestions as to where I can locate the certificate please.
Hi Rosie,
I'm sorry for your loss. My wife passed away recently, but it was in hospital so slightly different.
She passed on a Friday, and the MCCD was done by the ward doctor. This was sent electronically to the ME, who rang me on Monday to confirm the cause of death. They then sent the MCCD to the registry office, and I made an appointment with them to register the death the following Friday. They then give you any death certificates you've ordered (you don't get/see the MCCD), and the "green form" for either burial or cremation.
My wife was taken directly from the hospital to the chapel of rest at the funeral director's (FD) on her passing, but that was only because the hospital didn't have a mortuary. I don't believe you need any paperwork for your husband to be moved to the FD.
You don't have to speak to the ME if you don't want to. If the MCCD has indeed been sent to the registry office, you could call them to confirm they have it, then book the appointment to register the death.
Hope this helps.
Andy
You can often get a medical certificate online, which might speed things up if you're struggling with delays. Some services let you request it digitally without having to go in person, which can be a relief during such a tough time. If you're unsure where to start, the GP’s office should be able to clarify the process and whether a medical certificate online is an option in your area.
