Carboplatin side effects alongside immunotherapy

I have incurable metastatic gynacaelogical - 2 lymph nodes and peritoneal deposits - endometrial carcinoma - though had hysterectomy 2004 due to ovarian cancer. Having dostarlimab alongside carboplatin every 3 weeks.

Having problems with side effects that are not too bad immediately after treatment but come with a vengeance on day 6.  Mainly diarrhoea, bloating, hot flushed feeling in abdomen and feeling grim. Tiredness daily.

I have ileostomy and crohns history (though not recently) so am scared about inflammation.  

I have had 4 out of 6 cycles but due 5th in 2 days. I'm debating in my head following consultation today whether to hold off immunotherapy this week.

Having tead all the threads under Immunotherapy going back a year to last month im confused. Side effects with Dostarlimab are not bad according to people who have had it and commenyed. Any thoughts appreciated.

    I am sorry to hear that you are having a rough time with your treatment. 

    I had chemo in 2022- paclitaxel and carboplatin. I have not had immunotherapy but there are ladies on here who are currently having it. 

    One thing to suggest is to speak to your oncologist about your steroids - I had prednisolone and initially I was on it for the first 2 days after chemo day. I would feel not too bad and then I would have side effects that would kick in as soon as the steroids stopped. My consultant felt that the sudden drop from a high dose to nothing was exaberating my symptoms. From the second cycle she put me on a tapering dose. So it would be high doses on the hospital day and for the next 48 hours. Then my dose would halve on the third day after chemo and the halve again on the fourth day after chemo. It made a big difference- particularly with tummy effects. 

    I can understand your concerns about the inflammation with your crohns history and ileostomy. 

    It is always difficult to make decisions about treatments- why not give the Support Line a call tomorrow and talk it through with one of the nurses.

  • Further to my thread, I have just spoken with my oncologist who had similar thoughts and has agreed to reinstate steroids, which were reduced due to troublesome indigestion after a previous treatment. That worked regarding indigestion but increased a loose bowel issue. She will prescribe tablets for the indigestion instead.

    I note your advice re slowly reducing steroids if necessary and will be aware for future.
