No further treatment

Hi

I was wondering about the variation in giving further treatment for stage 1a high grade cancer following hysterectomy.

I had serous type cancer found in a small polyp after hysteroscopy. So stage 1a. After hysterectomy nothing else was found so it remained 1a. The surgeon gave me the news over the phone whilst I was out so I couldn't really ask any questions. The letter I received stated that everything had been removed before I even had the hysterectomy, at the time I had the hysteroscopy.

I have a follow up appointment in July but I haven't had a chance to talk to anyone about the results.i have requested this,  but no- one has contacted me.

My isuue is that i know the high grade womb cancers have a higher rate of distant recurrence, and there is considerable national variation in treatment. I.may have asked this before so apolgies! But has anyone had stage 1a serous type and not been offered adjuvent treatment?

  • Mine wasn't serious type, but it was stage 1b, grade 3 and I had the POLE mutation so have not had further treatment x

    Mine was stage 1b carcinoma sarcoma. It is also a high grade endometrial cancer so comparable to serous types. (There are several different high grade types)

    I had chemotherapy and external beam radiotherapy. At my hospital all high grades- whatever stage do get offered adjuvant treatment. 

    There is variation between hospitals however.

    I am glad I had the treatments as I have done everything possible to reduce recurrence.

    July is a long time to wait and worry so I would perhaps consider emailing the consultants secretary and requesting someone call you. I did this a couple of times. Once the consultant called me back, the other time it was one of the CNS team.

  • Thanks B74. I have read about the POLE mutation on here. I'm assuming I don't have this mutation- nothing was said about it.

  • Thanks Jane. I will try again to get in contact. But the policy where I live seems to be not to offer further treatment to all people with high grade cancer and the decision has already been made . I will try to share my concerns with the appropriate people but I don't think it will influence their decision. Actually I remember now at my first appointment, my consultant said I would only be offered further treatment if they found anything after hysterectomy.

    From what I've read , even stage 1a covers different things. It can mean the cancer is in the womb lining but hasn't spread outside the womb lining. But contained in a polyp this is seen as even less invasive as it isn't anywhere else in the womb lining. Perhaps that's why i haven't been offered anything.

  • There certainly does seem to be a wide variation. I was assessed Stage 1a Grade 1 before my operation and that stayed the same post operation. However I was offered brachytherapy because they found LVSI in a MELF pattern within the womb which increases the likelihood of reoccurrence  I took up the offer and am halfway through my four sessions. Mine cancer  wasn’t the serous type. 

  • Hi I am at Christie’s cancer hospital in manchester and stage 1a high grade also, they said they go off international guidelines and that it has hardly left the lining of the womb so woukd have 3 sessions of bracytherapy which I start next week, they said if it was in the womb lining I would have had no bracytherapy.

    I also didn’t have a face to face as they told me over the phone as I had to wait nearly 5 weeks due to Easter but I did discuss it further at the next app to discuss treatment which you will have missed like Jane said I would call the surgeons secretary or nurse specialist  and ask for an appointment, also are you at a different hospital than yoir original appointment like I was referred from my local hospital, so could you go back to the nurses there and ask for a gyny appointment as they would have attended yoir MDT xx

  • Thanks Shirleyana. Yes i am at the same hospital as my original appointment. I have a follow up appointment for July and it is with the same consultant, who was also my surgeon. I don't think I'll get an earlier appointment as they have decided to just give me 3 monthly check ups and no other treatment. But i am going to try for a phone conversation to get more explanation.   • Hi Sorrell, I was diagnosed with stage 1a grade 3 serous endometrial cancer in July 2022. In my case, the cancer was in a fibroid. After my total abdominal hysterectomy in October 2022 I was told that the cancer was contained within the womb and had not spread. My surgeon asked if I'd like to talk to the oncologist about any further treatment and I agreed to this. He advised me to undertake brachytherapy to reduce the risk of the cancer returning. I agreed to this and, thankfully, all my follow-up checks and scans have been clear. If you have any queries or concerns about any further treatment it might be helpful to talk to your surgeon before your July appointment. Perhaps you could e mail your CNS and asked if this would be possible?
