I'm a 74 year old male, who was diagnosed with advanced prostate cancer in May 2025. My PSA level at that time was over 300.
On 25 May, 2025, I had an MRI and my urologist informed me the cancer had spread to bone and lymph nodes. He started me on a double Firmagon injection into the stomach on 30 May 2025 and one injection a month afterwards.
On 09 June, 2025, a biopsy was done that confirmed the urologist's diagnosis.
On 24 September, 2025, my PSA level had reduced to 21. The Firmagon injection was discontinued and I was started on Docetaxel chemotherapy (a 1 hour infusion administered every 3 weeks over 18 weeks - 6 sessions in total).
On 05 November, 2025, I commenced to receive an Xgeva injection, one every 3 months for bone strengthening. On 14 November 2025, I was prescribed Relugolix 120mg tablet, taken once a day, to lower testosterone.
On 19 December, 2025, I was prescribed further hormone treatment with Darolutamide (4 x 300mg tablets/day
On 07 January 2026, I finished chemo and my PSA level was at 2.07. I now continue with Relugolix and Darolutamide tablets and the next Xgeva injection is due on 05 May 2026.
On 09 February 2026, I had a CT bone scan done and was told by my oncology team they were happy with my progress but they ordered a PSMA-PET scan because my PSA was 2.7.
On 28 April 2026, the PSMA-PET, unfortunately, revealed that the some cancer was still active.
Retesting the biopsy done on 09 June 2025 and BRCA testing will now be done to determine future treatment.
Needless to say I'm disappointed. As my situation is current, if there are others in a similar position I would very much appreciate to hear to hear from them, and hopefully exchange information and give support.

A warm welcome to the prostate cancer forum, although I'm sorry you have the need to join.
My prostate cancer was first diagnosed in 2016 and I had RT plus 7 months HT. It then recurred in early 2024 and I started similar treatment to yours, 3-monthly prostap injections, docetaxel chemo and darolutamide which is working OK for me at present.
I'm not surprised you're disappointed with your current position having undergone all the previous treatment, especially docetaxel, but I'm sure once the BRCA test is complete you'll soon be on your way to further, hopefully more successful,treatment.

I said I would get back to you - I've been out with a few mates in licensed premises. Well even people with cancer can try and lead a normal life.!!
I agree with Derek - you've been "through he mill" however you look to have a decent team looking after you, so there are further treatments plus targeted radiotherapy if they can identify the hot spots.

Thank you, and you surely are a man of your word even after visiting a licensed premises!
I did read through Derek's report by clicking on the excavator. Reading both of your reports gives me good encouragement. I just need to keep active and positive over the next six weeks.
I suppose I'm somewhat lucky to have few side affects from all the treatment. The only lingering effects are a slight tingling in the finger tips and numbness around the toes.
I will certainly keep you informed on my journey and I will try to make a better chronology of important events.

I am also a PCA patient. I have undergone prostatectomy, salvage radiation, Adt with relugolox and abiraterone. None worked for long. Real surprise was the ADT dint give me a remission of even 1 year. Within 8 months my psa increased. I too developed bone pain and psma pet scan showed bone mets in c6.
I was given SBRT radiation at curative dose. Ppst that my docetaxel chemo started and i habe completed 6 cycles..as of now psa is negligible.
But given my history of biochemical recurrences, i am not very hopeful of any long term remission from chemo.
I have discussed with my doctor about future trestment and they are Lutetium 177 and cabazitaxel if my cancer remians psma avid. If it transforms to neuro endocrine then only option will be carboplatin chemo.
At 57 years, my treatments got exhausted real fast..
