Hidden prostate cancer

Hi,

Following a couple of months of quite severe back and joint pain, I was diagnosed last July (2025) with advanced stage prostate cancer. The back and joint pain is caused by the cancer having metastasized to my bones and lymph nodes.

PSA levels were up close to 4000 at the time, so I was immediately started on hormone therapy (degarelix) injections once a month. Subsequently daily enzalutamide tablets have been added to the mix and currently my PSA levels seem to have stabilised in the thirties, so still quite elevated.

The odd thing is that I had the standard digital exam but 2 different doctors stated my prostate felt normal. I had an MRI of the prostate and no problematic areas were found and then I had a biopsy of the prostate which came back all clear.

The bone cancer is real, confirmed by various scans, so the only thing that really explains my condition is prostate cancer, it’s just doing a very good job at hiding itself.

I’d be curious to know if anyone else is in this situation or knows of someone else who has similar symptoms?

    sorry to find you here, but welcome to the forum. I think it’s called “occult” prostate cancer. Possibly started somewhere on the edge of the prostate, but migrated out the side and into the bone. I did read about it some time ago. My husband had no sign of anything with his exam even though he knew something was wrong for sometime. Reason was it was growing on the other side and took a while to work its way round. It tends to grow on the side of the rectum first so easier to detect early. So not that similar in that it was in the prostate before it broke out.

  • Hi  , that’s a remarkable story.  The biopsy detecting nothing, I guess means that there is a very small core, undetected from the samples and MRI.  Have you had a PSMA PET scan to identify where it is lurking?  Has the pain subsided after HT?
