Prostate Cancer treatment options - thoughts?

Hi all. Can anybody out there just throw me some advice.

I'm on AS, but my cancer has gone from Stage 1 gleason 3+3, to Stage 2 gleason 3+4, in the space of 2 months. They have now given me the choice of the 4 options:

  • 1: Stay on AS
  • 2: Hifu which only treats the area that is Stage 2  
  • 3: Prostatectomy  
  • 4: Radiotherapy

I really want to go down the Radiotherapy route but I'm 8 weeks out from a total hip replacement, and I'm hoping in the new year to have both my knees replaced which are in a bad state. I've had a couple of remarks saying it wouldn't be possible due to Radiotherapy giving issue to bones. Is this so, my new hip is ceramic and titanium and I'm guessing my new knees would be similar too would that make a difference.

  • Hi Hugster and welcome I would stay on AS for now as you have enough other stuff going on.

    But how do they they the different Gleeson in months , have u had 2 biopsies?

    When I say stay on AS I am assuming PSA not too high and tumour size not too large and not near the gland edge.

  • Hi Steve. Yes the first area was detected through the standard biopsy, and the second higher area through the template biopsy. Had my PSA blood test 2 days ago results hopefully today. When I say standard biopsy as an old school rugby playing, boxing man's man there was nothing standard about it, if you know what I mean lol.

    A warm welcome to the Online Community although I am so sorry to find you joining us.

    Let me at the outset say I am not a fan of AS and as you have a rising Gleason Score and no doubt increasing PSA it's time for action. The cancer isn't going away.

    There shouldn't be any issue with radiotherapy - a friend of mine with a ceramic and titanium hip has had it done and they said it was no problem. (he's had one knee done too!!). 

    Your team have not given you one other option which may well be suitable and that's brachytherapy. It may not have been offered as your hospital isn't one that does it, however if it's suitable you can have it at the closest Trust where it's offered. Here's a link for you:

  • Hi  , you don’t say what age you are?  Has HT been mentioned? I am sure we have answers to RT after hip surgery but will leave that to the experts.  If you create a profile it helps contain all your info in one area.  You can view ours by clicking on a name or avatar.

  • Hi David age 60. Yes HT was mentioned alongside the Radiotherapy. I also have 2 small tumours on left side of head, one sitting on optic nerve which is why I'm totally deaf in left ear. The other is in the actual choclear of the left ear which is a bit rarer and will possibly also need Radiotherapy. People saying yes but PC can kill but dodgy knees can't but the knees are miserable and affect my life. It's a total head fcuk (pardon my French).

    Just a thought (& I'm not medically trained), but if you do go down the HT/RT route, it's likely that you will be on the HT for 6 months before any RT would start.  If you are having your knees sorted in the New Year, this may well enable you to have both treatments.

    The HT should put the prostate cancer to sleep & stop any growth, & having the knees sorted would help with immediate quality of life.
