PSA rising, now starting Cabazitaxel

My husband was diagnosed with metastatic prostate cancer in November 2024 at the agecof 59 - T4 N2 M1, PSA 312.

He completed Triplet Therapy in May 2025 and at its lowest his PSA was 4.6. Over the last couple of months his PSA has risen to 26 and he is due up start cabazitaxel next week - 10 cycles in total.

He has been told to stop taking  Darolutamide as it is no longer working but will continue with his monthly hormone injections along his chemotherapy. 

Does anyone know whether the Darolutamide should be replaced with another drug once your cancer has become castrate resistant?

Any advice would be great.

I'm unable to give any advice to your query but I to finished triplet therapy in July, I was also 59 when diagnosed,  as im only a couple of months behind your husband could I ask were the cancer has spead to . I no this is of no help to you but it may give me an idea of things to come. stay strong, regards dale

    Sadly it has spread to the hips, femur, ribs, shoulder and base of the spine. He had no symptoms other than a back ache for a week when he was diagnosed.

  • Hi Cornwall12, depending on circumstances, and if you haven't been on them before, Abiraterone and Enzalutamide, in that order, though your cancer can build up a resistance on Abiraterone to Enzalutamide as though they are different treatments, they both target the AR pathway, you may go straight onto Enzalutamide, but this will likely rule ot Abiraterone as results are negligible, whereas the other way round some get a good response.

    Are these drugs normally taken whilst having chemotherapy?

  • Abiraterone used alongside cabazitaxel has shown good results, the results of using Enzalutamide with cabazitaxel are inconclusive and the combination is a very powerful one which can be difficult to tolerate so a sequential approach is standard, and more effective.
