I’ve had two mrI first biopsy trus biopsy negative  now want second biopsy 3D ultrasound  biopsy PSA steadily rising now 11  urologist said second biopsy showing pirad  four but wants another 24 samples I’m really worried why not just target the pirad 4 and no other areas? 

  • I don't know, though there's a couple of possibilities, what did the urologist say was the reason.

    I understand your questioning this , I went thru similar.

    What did the MRI show in terms of tumour size and location in gland, ie: near the capsule border/edge?

    There are a couple of other suspicious area maybe 2 but because we are taking the samples we might as well take all and more was the reason I felt

    Pirad 2 sorry and pirad 4 near centre 

  • I had a Trus biopsy originally, then went on active surveillance for 4 years.

    I had 2 slow growing tumours.

    They then wanted me to have a perineal biopsy which I didn't want.

    Some months later started on Radiotherapy and another 4 years later things r ok.

  • I think that makes sense, they're just being thorough. The more comprehensive your assessment is,  the better. That way in future they'll never have to ask "is this new or has it been here since the beginning?", Which could influence treatment decisions down the line.
