I had my prostrate removed in July and my first psa check came back at 0.53 way higher than the 0.01 that is normally expected. My original Psa was 26 so I guess maybe it will just take time to drop. Any thoughts or suggestions are welcome
Hello Herdie. How long after your prostatectomy was your PSA taken? Since your pre prostatectomy PSA was 26, you probably just have some risidual benign tissue left over. I had that happen to me. My second PSA was .04. Good luck to you!!
Hello. I am a 62 year old and was diagnosed with Gleason 7 (3+4) prostate cancer last year (3-2024). I had a radical prostatectomy 10-2024. Pathology report showed 1 small positive margin with Cribriform architecture. No EPE or SV involvement.
Hi, Herdie, sorry your PSA has not dropped to undetectable after your prostatectomy. Looks like it has been at least 8 weeks since your surgery. This is normally enough time for the psa to reach its low point. This low point is called your nadir. After prostatectomy, your nadir should be less than 0.1 (<0.1).
Your care team should schedule another PSA test in a few weeks to confirm the reading.
If confirmed you will need an oncologist to discuss follow up treatment.
I have another psa check on 27/10 and a follow up with my surgeon for early November and I guess we will see what the results are
Hi it was after 8 weeks . I have another psa check psa check on 27/10 so I guess we will see what that is .
Hi Herdie1967 don’t panic, although the nadir (lowest PSA) is usually within 4 to 6 weeks after RP, there are studies suggesting a later nadir can have better outcomes. Hopefully your PSA is still falling on your next test.
Best wishes, David

Latest Psa check was 0.57 so it’s going up . Next week have an appointment with my surgeon.
Hi Herdie1967 , that isn’t what you hoped for but actually fairly stable as it has only risen by 0.04 in 3 months. Try not to over think this, hopefully your surgeon can offer advice when you meet and put you in touch with an oncologist.
Best wishes, David
