Hi,
For some years now I've been home testing for PSA and Free-to-Total PSA Ratio, and recording the results in a health spreadsheet. I've used the testing kit from Lets Get Checked (processed by The Doctors Laboratory), however they've now dropped this test from their product range.
Having the FTT stops me overthinking minor fluctuations in an otherwise normal level, but also I just want to maintain a history of results with the same detail as to date.
So my question, does anyone know of a test kit that includes in the report Free PSA and FTT ratio, even for low Total PSA?

Hi Cyy welcome to the forum. I can't answer with any ideas about where to purchase such an item..However, my friend runs a local group Prostate Buddies and he and I have had many conversations about PSA levels. It appears that it is the start of the diagnostic process but like many other scans etc it is not 100% reliable and can be affected by many things. If you don't have Prostate Cancer (have I understood that right) then surely testing so regularly will cause a degree of anxiety if there are any variations. Hi Cyy , I don’t have any experience with self testing, but noticed that you hadn’t had many replies. I completely agree with Gail (GRANNY59 ), you might be over worrying by frequent testing. My suggestion if you want to keep testing is look at trends over a minimum of 6 months but generally a year. I don’t know what your PSA start point is, or your age for that matter so can’t give you help on that, but I am sure if you post those on here we can give our opinions. Do you have any symptoms? I didn’t and my PSA was 74 at the start. David

For good or bad, I'm both intensely analytical and intensely pragmatic, and it affects everything I do. Thank you for your concerns but honestly I feel no more anxiety about PSA testing than monitoring my weight or BP. Initially I tested each 8 months or so to get a baseline and it's been ad-hoc since, my most recent test was Feb '24, so 16 months.
When I looked into this originally I concluded that the binary pass/fail kits were not for me. From what I've read I think it's useful to build a history of Total PSA and Free PSA / FTT ratio. That last part (Free PSA) seems unavailable now.
Some of the providers will report FTT if Total is elevated. I do wonder if there's a clinical reason for not routinely reporting FTT, though I suspect it's cost cutting in a competitive market.
As an aside, I read the TRANSFORM clinical research trial backed by the NHS and claimed to be "the biggest trial in prostate cancer screening for 20 years", will include PSA screening, with invitation letters going out "later in 2025". https://prostatecanceruk.org/research/transform-trial.
