Hi,
My husband is advanced prostate cancer with nodes and bones in the pelvic area, gleeson 8. He is on Enzalutamide and 3 months Zoladex. He react well, PSA dropping nicely.
Yesterday we have seen the ecologist and discuss RT which will start later in the spring. I was expected 20 sessions in 4 weeks and it is not the case. He will have 6 sessions over 6 weeks. I researched it last night and apparently this is the protocol for advanced cancer. The RT will be only to the primary ie the prostate but not to the bones or nodes in the area.
My question: Anyone had the same pathway of only 6 sessions?

Hello jacobanddafna That sounds like he's going for SBRT (Stereotactic Body Radiotherapy). This is a fairly new treatment where you have fewer stronger doses - they are longer fractions over a longer period. They are new RT machines being rolled out but not everywhere has them.
I am sure we have some forum members who have "been there done that" so I hope you get some replies.

In ''normal'' 20 sessions RT, do they also mark the area with metals markers follow by tattoos or is it only with SBRT?

I know they give you the 3 tattoo's with SBRT and the External Beam Radiotherapy - I am proud of my 3 "dots" they are the only tattoo's I have and came free on the NHS . During my Radiotherapy my Radiologists were quite happy to enhance my tattoos with black felt tip too!!
I read somewhere that the "Laser Placement" is to be introduced for Radiotherapy but I am not up to speed on that - if anyone knows can you please add the details.

Yes they do. I got to age 72 without any tattoos!
Hi Daphna, usually having so few RT sessions with advanced prostate cancer is to target the bones to reduce pain, I have been offered 5. as your RT is not for this, then SBRT, as Brian says is very likely. Daphne 6 sessions of SBRT is as effective as a standard RT. course, take care.

Can I put some more detail on this. There is an article in the Lancet which suggests that the 6 sessions could be 36GY for 6 sessions over 6 weeks (6GYper session). This is an alternative to the 60GY over 20 sessions (3GY per session).
