My understanding for what it's worth

37 PSA psa-d 1.28/volume 29 ml - You have an elevated amount of PSA in the prostate for it's size which shows that cancer is behind the density not benign prostate enlargement.

right pz lesion - pirads 5 - no nodes -  The MRI is PIRADS 5 which means there is a high likelihood of clinically significant cancer - the cancer is in the right side of the prostate. The good news is none has been found in your lymph nodes.

No T3 Gleason 7 (3+4) - Again good news No T3 means the cancer is all within the prostate - it's not gone "walkabout" in your body. Of the samples examined 65% were pattern 3 and 35% pattern 4 the more aggressive cells - this gives you your Gleason score.

In kitchen fitting terms the above means "it will take some time to get it sorted, it needs a good dose of looking at but we have the tools and skills and we can fix it".

The above is my not medically qualified understanding based on my own personal experiences.

Hi Pompeyblue , Brian has given you the definitions and a great analogy.  My take is that you have been caught in the very early stages of a not very aggressive and slow growing cancer.  You should be able to be cured, so your kitchen skills will be used for a long time. There are lots of options for you, so don’t rush into anything.  I am fairly sure the PSMA scan won’t show anything worse, but best find out now.

Please come back with any questions. We were all at your stage in our knowledge at the beginning of our journeys.  I would add for you and others reading this, that treatments have and still are improving year on year.

I can't add anything relevant to the replies you have already had from Millibob & David2017, but wouldn't it be fantastic if alongside the detailed medical results hoispitals could add a laymans (or in my case an idiots) guide to interpret?   I doubt if this will ever happen though as it might remove some of the mystery behind the medics.

You are lucky you have all this information. We were just told PIRADS 5 T3a Gleason 3+4. No further information forthcoming. 

If you are in the UK, then you can get all this information from your GP, either directly or through the NHS app.

The consultant updates the GP via letters, which are usually freely available on the app. If they are not then you usually get them added to what you see. There is a possible restriction in the app which the doctor’s surgery can remove.

Most of these reports are summarised in medical speak. I copy the results results in an AI app to get an explanation and then, because I like to check, I google the document line by line.

i have always, to date, found the AI summary to be very accurate.
