Help deciphering consultant’s feedback

I wondered if one of the many experts here might help me make sense of what my consultant fed back to me today?

So, apparently I have PC that’s got a Gleeson score of 3,3. However, they got an ‘indeterminate’ reading off one lesion. They’ve asked to do another biopsy next week.

She said they need to decide whether to monitor or whip the prostate out. If it’s a 4, then the surgeon said they’d recommend removal. I’d like to avoid surgery if it’s safe to do so though.

The prostate is 56ml, my PSA is 6.3 and I’m 56. The cancer cells are all within the prostate.

My wonders are that I don’t know if this is standard advice; how at risk I am or what my outcome means contextually.

Is it better to watch and wait?

Sounds like they are a bit uncertain of the Gleeson score, not heard of that before so potentially raises a few eyebrows.

Anyway sounds like you are ok with a low PSA and cancer contained, don't forget there is the other option of Radiotherapy, have they not suggested that?

One thing worth checking is the tumour size in mm cos if u were to go on AS you don't want it too big obviously.

I suggest that you read up on Gleason scores here:

For gleason 3+3, the default treatment path is active surveillance (AS).

For a gleason score of 3+4, the options are typically AS or intervention.

For a gleason score of 4+3 and higher, intervention is normally required.

There may be factors, which are not apparent from the info that you have provided so far, which influence these options.

At this point you are not really in a position to make any firm decisions, you will have to wait for the outcome of the next biopsy.
