I am on holiday in central America flying home tomorrow
I had to stop taking apalutamide while on holiday because of severe rash which is still there but much less
I am due to start 20 lots of radiotherapy on 27th
Will the fact I have stopped apalutamide, still on zoladex affect my radiotherapy?
I will contact cancer unit while home
Hi Mal. Safe journey home tomorrow and glad that the rash is reducing. There is no relationship between having to have hormone therapy in order to have radiotherapy. The way the UK works is that they like to give HT to reduce the testosterone levels and shrink the tumour before offering RT in order to make a smaller target and reduce collateral damage. The reason you were put on both Zoladex and Apalutamide is that they give a more complete stop to the production of the testosterone which feeds the cancer and has been shown to provide a longer time to possible progression compared to Zoladex on its own. They might suggest a different second generation antiandrogen. You have already reached the target for the experts to put you forward for the radiotherapy and had your tattoos so my personal opinion is that you are good to go. My husband had his radiotherapy when he was only on Prostap which is also an injection.

From what I know Apalutamide is a inhibitor in the sence that it blocks the prostate cancer cells possibility to feed on Testosterone. Zoladex is the actual Testosterone suppresser making sure that your testicles stop producing Testosterone which is the fuel for prostate cancer.
So if you still tolerate Zoladex that’s the biggest success factor because there are lots of studies showing the increase of efficacy with the combination of radiation and hormone therapy compared to radiation alone.
So of the 2 medications the most important one you’re still on; Zoladex
There are other 2’d gen medications that they can try instead or they could try to add Abiraterone because then we are talking total Testosterone suppression.
But like the other very good people in your thread says, your Zoladex is working so it should be no worries in time for the Radiation Therapy
