Pancreatic adenocarcinoma enzyme replacement therapy not working

My best friend was diagnosed very recently with with adenocarcinoma of the head of the pancreas which at present is unresectable. She needs to start chemo in January if she has any chance of reducing the mass for possible surgery.

Here lies the problem:

She has been taking PERT for several months now with no effect even after changing brands and increasing her proton pump inhibitor to the maximum dose. She continues to try to eat despite nausea and vomiting and in excess of 15 bowel movements per day. She is also diabetic and is getting so little nutrition that she has frequent hypoglycemia and no need to inject insulin. She is loosing around 1kg every 10 days (so far over 20kg since July).

If anyone has had a similar experience and has any suggestions I would love to hear your views.

I fear she will die from malnutrition before even getting a chance to fight the cancer
