I am stage 3 melanoma, having a primary removed from my shin 3years ago, and a lymph nodes disection a year ago. I think I can feel a lump, and pea size nodgels, in my elbow crease ,I don't know if I'm being paranoid, but should I be worried?

It's always worrying when you notice something new and the simple answer is, no, you're not being paranoid.
Why don't you give your SCNS a call and talk it through with her. If she thinks it needs checking she'll make you an appointment with your consultant.
